Fetal maturasyonda profilaktik deksametazon ve betametazonun fetal kalp hızı paterni ve biyofizik profili üzerine olan etkileri

Özet
Amaç: Fetal akciğer maturasyonunu arttırmak için kullanılan antenatal deksametazon ve betametazon tedavisinin, fetal kalp hızı aktivitesi ve biyofizik profil skoru üzerine olan etkilerini araştırmak ve kıyaslamaktır. Yöntem: Preterm eylem tanısı almış 28-34. gebelik haftaları arasındaki 100 hasta prospektif olarak çalışmaya dahil edildi. Randomize olarak seçilen 50 hastaya deksametazon 6 saat arayla 6 mg (toplam 24 mg) (Grup I), 50 hastaya betametazon 12 saat arayla 12 mg (toplam 24 mg) (Grup II) uygulandı. Hastalara uygulama öncesi ve uygulamadan 24 ve 48 saat sonra non-stres test ve biyofizik profil skoru değerlendirmeleri yapıldı. Her iki grupta da, tüm parametreler için bazal değerlerin ortalamaları ile 24 ve 48. saatlerdeki değerlerin ortalamaları tek tek karşılaştırılırken buna ek olarak gruplar da kendi aralarında kıyaslandı. Bulgular: Her iki grupta da fetal kalp hızında değişiklik gözlenmezken, akselerasyon sayısında ve variabilitede anlamlı bir azalma saptandı. Ancak akselerasyon sayısındaki azalma Grup I’de daha belirgindi. Her iki grupta de biyofizik parametrelerden fetal solunum hareketleri ve fetal vücut hareketlerinde belirgin bir azalma saptandı. Ancak, Grup I’de fetal solunum hareketlerinde, Grup II’de ise fetal vücut hareketlerinde 24. saatte anlamlı bir farklılık gözlenmedi. Grup I’de non-stres testte belirgin bir nonreaktifleşme saptanırken, Grup II’de anlamlı bir farklılık gözlenmedi. Toplam biyofizik skorlara bakıldığında her iki grupta da 24 ve 48. saatlerde belirgin bir supresyon gözlendi (p=0.0001). Sonuç: Hem deksametazon hem de betametazon, fetal kalp hızı paterni ve biyofizik aktivite üzerinde belirgin supresyona neden olmaktadır ancak akselerasyon sayısı ve non-stres test üzerindeki etkileri göz önüne alındığında betametazon ilk tercih ajan olarak önerilebilir. Ayrıca fetal hipoksi olarak değerlendirilen sonuçların da verilen steroid ile ilişkili ve reversibl olabileceği akılda tutulmalı, erken dönemde fetal distres tanısı ile acil doğumun düşünülmesi durumunda iatrojenik prematürite oranlarının yükselebileceği unutulmamalıdır.
Abstract
Objective: To evaluate and compare the effect of antenatal dexamethasone and betamethasone therapies that are used to accelerate fetal lung maturation on the fetal heart rate activity and biophysical profile score. Methods: One hundred patients with the diagnosis of preterm labor in their 28-34th gestational weeks were enrolled prospectively. A course of dexamethasone of 6 mg in 6 hour-intervals (24 mg totally) were applied to randomly chosen 50 patients (Group 1) and 12 mg bethamethasone in 12 hour-intervals (totally 24 mg) was given to the remaining 50 patients (Group 2). The patients were evaluated with non-stress test and biophysical profile score before, at 24th and 48th hours of application. The basal means of all parameters were compared with the means of 24th and 48th hours one by one and also the groups were compared with each others. Results: Although there was no change in fetal heart rate, significant decrease in acceleration number and variability were noted in both groups. The decrease in acceleration number in Group I was more prominent than Group II. The fetal respiration number and fetal body movements which are the parameters of biophsical profile score were similarly decreased in both groups; however, at 24th hour fetal respiration number in Group I and fetal body movements in Group II were not differing significantly. An obvious nonreactivity was determined on non-stress test in Group I but not in Group II. There was a significant suppression in total biophsical profile score in both groups at 24th and 48th hours (p=0.0001). Conclusion: Dexamethasone and betamethasone both cause prominent suppression on fetal heart rate patterns and biophysical activities; however, when their effect on number of accelerations and non-stress test were taken into account, betamethasone may be suggested as the treatment of choice. It should be kept in mind that the results being interpreted as fetal hypoxia may be associated with the type of the given steroid and therefore is reversible, so iatrogenic prematurity rates would increase significantly if emergent delivery is to be considered due to the diagosis of fetal distress.
