Düşük tehdidi sonrası dördüz gebelikte fetal redüksiyon

Özet
Amaç: Çoğul gebeliklerde fetal redüksiyonun uygulandığı ilk trimesterde (10-13. haftalar) erken uterin kanama ve düşük tehdidi önemli problemler olarak karşımıza çıkabilmektedir. Bu çalışmamızda, abortus imminensli bir dördüz gebeliğe uygulanan fetal redüksiyon sunulmakta ve tartışılmaktadır. Olgular: 28 yaşında, 3.5 yıllık infertil hastaya human menopozal gonadotropin ile yapılan ovulasyon indüksiyonu sonrasında 9. haftadaki ultrasonografik değerlendirmesinde dördüz gebelik olduğu tespit edildi. Embriyo redüksiyonuna karar verilen hastada hospitalizasyonun 2. gününde spontan vaginal kanama başladı. Ultrasonografik değerlendirmede 4 fetus da canlıydı ve servikal uzunluk 4 cm, servikal kanal kapalı ve servikal kanal iç yüzünde 28x41 mm’lik hematom ile uyumlu oluşum izlendi. Bir hafta sonunda kanaması kesilen hastada 2 fetusa 2 gün arayla redüksiyon uygulandı. İşlem sonrasında kanaması olmayan hasta 14. gebelik haftasında taburcu edildi. Hasta gebeliğin 38. haftasında travayın başlaması ve ilk fetüsün makat prezentasyonu nedeniyle sezeryanla ilk fetüs; canlı, kız, 2000 g ve ikinci fetüs; canlı, erkek, 2400 g ve Apgar skorları normal olarak doğurtuldu. Sonuç: Düşük tehdidi olan çoğul gebeliklerde fetal redüksiyon mümkün olduğunca erken yapılmalıdır. Ancak ciddi düşük tehdidi olan gebelerde fetal redüksiyon hala tartışmalıdır ve bu konuda kapsamlı çalışmalara ihtiyaç duyulmaktadır.
Abstract
Background: Early uterine bleeding and threatened abortion appear as important problems in multiple pregnancies in the first trimester (10-13 weeks) that fetal reduction is applied. In our study fetal reduction in a quadruplet pregnancy after threatened abortion is presented and discussed. Case: After ovulation induction with human menopausal Gonadotropine, a 28-year-old patient with infertility for 3.5 years, quadruplet pregnancy was diagnosed by ultrasonography in the 9th week of the pregnancy. Since she has a quadruplet pregnancy, selective fetal reduction was decided, but spontaneous vaginal bleeding started at the 2nd day of hospitalization. By ultrasonography, 4 fetuses were alive, cervical length was 4 cm, cervical canal was closed and there was a formation of hematoma (28x41 mm) on the interior side of cervical canal. After a week, vaginal bleeding stopped and fetal reduction was applied to two fetuses by two days interval. She was externated at the 14th weeks of gestation without any vaginal bleeding. At the 38th weeks of gestation, labour began and she underwent cesarean section because of the breech presentation of the first fetus. First fetus; alive, female, 2000 g and second fetus; alive, male, 2400 g were borned with normal Apgar scores. Conclusion: Fetal reduction in multiple pregnancies with threatened abortion must be applied as earlier as possible but in pregnants with serious threatened abortion it is still controversial and comprehensive studies are necessary on this subject.
