Özet

Amaç: Bu çalışmada kliniğimizde yatarak tedavi gören hiperemezis gravidarum olgularında tiroid hormonlarının ve antitiroid tedavinin etkilerinin araştırılması amaçlandı. Yöntem: Bu çalışma prospektif olarak planlandı. Haziran 2000-Nisan 2002 tarihleri arasında kliniğimizde yatarak tedavi gören, öncesinde herhangi bir medikal problemi olmayan, hiperemezis gravidarum tanısı almış hastaların demografik verileri, laboratuar bulguları, uygulanan tedaviler ve sonuçları hazırlanmış olan formlara kaydedildi. Bulgular: Çalışmanın sonunda 44 olgu değerlendirildi. Yaş, gravida ve parite ortalamaları sırasıyla; 27.24±4.12, 3.04±2.25, 1.58±1.88 idi. Standart tedaviye yanıt alınan 31 olguda tiroid hormon profili normal sınırlarda iken, standart tedaviye yanıt alınamayan 10 olguda (%22.72) ise hipertiroidi bulguları mevcuttu. Endokrinoloji kliniğince konsülte edilen bu hastalara propiltiurasil tedavisi başlandı. Bu gruptaki hastaların hepsinde klinik olarak olumlu cevap gözlendi. Sonuç: Özelikle standart tedaviye yanıt vermeyen hiperemezis gravidarum olgularında, tiroid hormon profiline bakılması ve hipertiroidi saptananlarda antitiroid tedavinin eklenmesi klinik olarak anlamlıdır.

Abstract

Objective: To evaluate the thyroid function tests and efficacy of antithyroid treatment in cases diagnosed and treated as hyperemesis gravidarum in our clinic. Methods: The study was planned prospectively. Between June 2000-April 2002 patients with no abnormal medical history who were treated as inpatients with the diagnosis of hyperemesis gravidarum were included. Demographic, laboratory findings, treatment and the results were recorded on to the prior prepared forms. Results: At the end of the study period 44 cases were evaluated. The mean age, gravida parity, was 27.24±4.12 years, 3.04±2.25 and 1.58±1.88 respectively. Standard treatment was successful in 31 cases whom thyroid function tests were normal. In the remaining 10 cases (22.72%) the standard treatment was not successful and thyroid function tests were abnormal. After consultating with endocrinology clinics these patients were given propylthiouracil and the treatment was successful in all patients. Conclusion: When the standard treatment is not successful in treatment of hyperemesis gravidarum thyroid function tests should be investigated and in cases with hyperthyroid findings addition of antithyroid treatment clinically seems to be reasonable alternative.
