Kilo fazlalığı ve obezite tanısı almış hipertansif olgularda karotis-femoral nabız dalga hızı değerlerinin karşılaştırılması

Özet

Amaç: Nabız dalga hızı, periferik arter dalga formlarından yararlanılarak geniş arterlerin elastisitesini değerlendirmede kullanılan bir tekniktir. Bu çalışmada Dünya Sağlık Örgütü'nce tariflenen beden kitle indeksi (BKİ) sınırlarına göre, kilo fazlalığı (BKİ: 25-29.9 kg/$m^2$) ve obez (BKİ$\geq$30 kg/$m^2$) olarak kabul edilen hipertansif olgularda arteriyel duvar sertliğinin bir indeksi olan nabız dalga hızı değerleri incelenmiştir. Gereç ve Yöntem: Çalışmaya 26 obez ve 57 kilo fazlalığı tanısı almış hipertansif hasta dahil edildi. Nabız dalga hızı, karotis ve femoral arterler kullanılarak, online nabız dalga kaydına ve nabız dalga hızının otomatik hesaplanmasına imkan veren Complior cihazı ile hesaplandı. Bulgular: Her iki grup arasında (sırası ile kilo fazlalığı ve obez) yaş (p=0.43), cins (p=0.59), sistolik kan basıncı (151.92±14.13, 155.57±12.98 mmHg; p=0.26), diyastolik kan basıncı (87.71±11.95, 88.26±11.04 mmHg; p=0.84, nabız dalga hızı (12.96±2.63, 12.02±2.55 m/s; p=0.13) ve kalp tepe atımı (77.75±6.05, 78.19±6.76 atım/dakika; p=0.76) açısından istatistiksel olarak anlamlı bir farklılık saptanmadı. Sonuç: Karotis-femoral nabız dalga hızı değerleri farklı BKİ'ne sahip hipertansif hastalarda benzerlik göstermektedir.

Abstract

Aim: Pulse wave velocity is a tecnique in which large artery elasticity is estimated from analysis of the peripheral arterial waveform. World Health Organization and other international committees defined different obesity categories according to the body mass index (BMI). The purpose of our study was to compare the carotid-femoral pulse wave velocity in patients with essential hypertension with different BMI categories. Material and Methods: The study group included 83 hypertensive patients (BMI: 25-29.9 kg/$m^2$, 57 patients; BMI $\geq$30 kg/$m^2$, 26 patients). The pulse wave velocity was measured automatically with a Complior Colson device. Pulse wave velocity was calculated from measurements of the pulse transit time between two recording sites, the femoral and carotid external arteries. Results: There were no significant differences in the age (p=0.43), sex (p=0.59), systolic blood pressure (151.92±14.13, 155.57±12.98 mmHg; p=0.26), diastolic blood pressure (87.71±11.95, 88.26±11.04 mmHg; p=0.84), heart rate (77.75±6.05, 78.19±6.76 beat/min; p=0.76) and carotid-femoral pulse wave velocity (12.96±2.63, 12.02±2.55 m/s; p=0.13). Conclusion: The carotid-femoral PWV were no significant differences between patients with hypertension and different levels of BMI.
