Özet

Bu çalışmada 40'ı farenjit, 17'si sinüzit tanısı alan toplam 57 olguda, günde tek doz, 3 gün azitromisin tedavisi, 10 günlük konvansiyonal tedaviler ile karşılaştırıldı. Farenjitli ve sinüzitli olgular rastgele ikişer eşit gruba ayrılarak, birinci gruplara 3 gün süreyle 500 mg azitromisin/gün, ikinci farenjitli gruba 10 gün süreyle 4 x 500 mg/gün penisilin V, ikinci sinüzitli gruba ise 3 x 500 mg/gün amoksisilin + klavulonat tedavisi uygulandı. Klinik değerlendirmeler tedavi öncesinde ve tedaviden sonra 2., 5. ve 14. günlerde yapıldı. Mikrobiyolojik kültür örnekleri de tedavi başlamadan ve tedavi başladıktan 5 ve 14 gün sonra alındı. Farenjit olgularının tümünde etken Streptococcus pyogenes olarak saptandı. Azitromisin grubunda tedavi sonrası tüm kültürler negatifti. Penisilin V alan 20 olguluk grupta ise 2 olgu dışında, tedavi sonrası kültürler negatif olarak belirlendi. Ateş ve diğer klinik bulgular, azitromisin grubundaki 8 olguda 2. gün, 12 olguda 5. günde düzelirken, penisilin V grubunda 11 olguda 2. gün, 9 olguda ise 5. gün iyileşme saptandı. Sinüzit olguları ponksiyon uygulamasını kabul etmediklerinden, etkinliğin değerlendirmesi klinik ve radyolojik bulgulara dayanılarak yapıldı. Bu hastalardan azitromisin alan grupta 2 olguda radyolojik düzelme olmaması, başarısız sonuç olarak değerlendirildi. Amoksisilin + klavulonat grubunda ise 1 olgu dışında tüm olgularda klinik ve radyolojik düzelme saptandı. Azitromisin alan tüm hastalarda istenmeyen bir yan etki gözlenmezken, amoksisilin+klavulonat alan 1 olguda vajinit, 1 olguda ise ishal belirlendi.

Abstract

In this study, the efficiency of 3-day course of azithromycin therapy was compared with 10-day course of conventional therapies on 57 patients with upper respiratory tract infections. Forty pharyngitis and 17 sinusitis cases were randomized into two equal groups, and the first group was treated with 500 mg/day azithromycin for 3 days while the second pharyngitis group was treated with 4 x 500 mg/day penicillin V for 10 days, and the second sinusitis group was treated with 3x500 mg/day amoxicillin+clavulonate for 10 days. Clinical assessment was performed prior to treatment and 2, 5 and 14 days after the initiation of treatment. Samples for microbiological cultures were taken prior to treatment and 5 and 14 days after the initiation of treatment. Streptococcus pyogenes was found to be the sole causative agent in all pharyngitis cases. All post-treatment cultures in the azithromycin group and except 2 of 20 cases in the penicillin V group were negative. In 8 cases of 20 azithromycin group, fever and the other clinical symptoms disappeared within 2 days and in 12 cases within 5 days of therapy, while these numbers were 11 and 9 cases in penicillin V group, respectively. As none of the sinusitis patients agreed to have a sinus puncture, assessment of efficacy was based on clinical and radiological findings. In 2 of azithromycin treated sinusitis patients, lack of radiological improvement was evaluated as treatment failure. In the amoxicillin+clavulonate group, all patients except one were improved clinically and radiologically. When the adverse effects have been evaluated, there were no complaints in the patients of azithromycine group, whereas one patient complained from vaginitis and one complained from diarrhoea in amoxicillin+clavulonate group.
