Tüberkülozun serolojik tanısı için kullanılan immünokromatografik hızlı testin (ICT) değerlendirilmesi

Özet
Bu çalışmada aktif tüberküloz enfeksiyonu sırasında Mycobacterium tuberculosis tarafından salınan beş farklı antijene karşı hasta serumunda oluşan özgül antikor yanıtını araştırmak amacıyla kullanılan "Immunochromatographic Rapid Test'in (ICT-Tuberculosis, Amrad, Australia) değerlendirilmesi amaçlanmıştır. Aktif akciğer tüberkülozu tanısı almış ve klinik örneklerinden Löwenstein-Jensen besiyerinde M. tuberculosis üretilmiş 34 hasta ile kontrol grubu olarak 60 sağlıklı bireyden (33'ü PPD pozitif, 27'si PPD negatif) toplanan serum örnekleri çalışmaya dahil edilmiştir. Kültür pozitifliği altın standart olarak alındığında testin duyarlılığının %73.5 özgüllüğünün ise %88.3 olduğu belirlenmiştir. Sonuç olarak, ICT-Tuberculosis testinin tüberkülozun laboratuvar tanısında tek başına kullanılmasının uygun olmadığı, temel mikrobiyolojik yöntemlerin yanında laboratuvar tanısını desteklemek amacıyla kullanılabileceği öngörülmüştür.
Abstract
In this study, the Immunochromatographic Rapid Test (ICT-Tuberculosis, Amrad, Australia) which is used for the detection of specific antibody responses against five different antigens secreted by Mycobacterium tuberculosis during the active tuberculosis infection, was evaluated. Serum samples collected from 34 active pulmonary tuberculosis patients whom M.tuberculosis have been isolated from clinical specimens in the Löwenstein-Jensen media, and 60 healthy control subjects (33 PPD positive, 27 PPD negative) were included to the study. The sensitivity and specificity of the test were detected as 73.5% and 88.3% respectively, when culture positivity was accepted as the gold standard. Consequently, we suggested that the ICT-Tuberculosis test was insufficient to be used solely and could be used as a complementary test to the basic microbiological methods in the diagnosis of tuberculosis.
