HBsAg pozitif serum örneklerinde hepatit B virus DNA'sının saptanmasında polimeraz zincir reaksiyonu ve hibridizasyon yöntemlerinin değerlendirilmesi

Özet
Moleküler yöntemler viral hepatitlerin tanısında, replikasyonun belirlenmesinde, tedaviye yanıtın ve prognozun değerlendirilmesinde önemli bir yer tutmaktadır. Bu çalışmada, hepatit B virus (HBV) yüzey antijeni (HBsAg) pozitif olan serum örneklerinde HBV-DNA'sının araştırılmasında "nested" polimeraz zincir reaksiyonu (nPZR) ve sıvı faz hibridizasyon (SFH) yöntemlerinin değerlendirilmesi amaçlanmıştır. Çalışmaya alınan HBsAg pozitif 737 serumun %44'ünde PZR ile %31'inde ise SFH ile HBV-DNA pozitifliği saptanmıştır. Serumların 228'i (%31) her iki yöntemle de pozitif sonuç vermiş, 96'sı (%13) sadece nPZR ile pozitif bulunmuştur. Bu gruplarda HBeAg pozitifliğinin ise sırasıyla %46.9 (107/228) ve %17.7 (17/96) olduğu belirlenmiştir. Sonuç olarak, HBsAg ve/veya HBeAg pozitif olan hastalarda vireminin gösterilmesi için HBV-DNA saptanmasının gerekliliğini, bu amaçla daha duyarlı olan nPZR yönteminin yeterli olduğunu, ancak kantitatif değerlerin elde edilebilmesi için ek olarak hibridizasyon yönteminin de kullanılabileceğini düşünmekteyiz.
Abstract
Molecular techniques are important for the laboratory diagnosis of viral hepatitis, demonstration of viral replication, monitorization of the antiviral therapy and prognosis. The aim of this study was to evaluate two molecular methods, nested polymerase chain reaction (nPCR) and liquid phase hybridization (LPH) assay, in searching hepatitis B virus (HBV) .DNA in HBV surface antigen (HBsAg) positive serum samples. A total of 737 sera were studied and 324 (44%) and 228 (31%) were found HBV-DNA positive by nPCR and LPH, respectively. Two hundred twenty eight (31%) of the sera showed positivity by both of the methods, whereas 96 were found positive by nPCR alone. The HBeAg and anti-HBe positivity rates in HBV-DNA positive patients were detected as 38.3% (124/324) and 31.8% (103/324), respectively. As a result, the detection of HBV-DNA is necessary for the determination of viremia in HBsAg and/or HBeAg positive patients, and nPCR seems to be an appropriate sensitive method for that purpose. However hybridization assay should be additionally preferred in order to obtain quantitative values.
