Hastane izolatı stafilokoklarda metisilin direnç ve aminoglikozid modifiye edici enzim genlerinin multipleks-polimeraz zincir reaksiyonu ile araştırılması

Bu çalışmada, yatan hastalara ait çeşitli klinik örneklerden izole edilen stafilokoklarda metisilin ve aminoglikozid direncinin fenotipik olarak disk difüzyon yöntemi ve genotipik olarak multipleks-polimeraz zincir reaksiyonu (PCR) tekniği kullanılarak araştırılması amaçlanmıştır. Genotipik olarak aminoglikozid modifiye edici enzimleri (AME) kodlayan aac(6')/aph(2"), aph(3')-llla ve ant(4')-la genlerinin yanısıra metisilin direnç geni mecA araştırılmıştır. Çalışmada 19'u S. aureus ve 30'u koagülaz negatif stafilokok (KNS) olmak üzere toplam 49 izolat incelenmiştir. Disk difüzyon yöntemiyle oksasiline dirençli olarak saptanan 34 (%69.4) izolatın 33'ünde (%97) mecA geni pozitif bulunmuş, oksasilin direnci ile mecA geni varlığı arasındaki ilişkinin S. aureus'larda %100, KNS'larda ise %95.7 düzeyinde olduğu gözlenmiştir. AME genlerinden aac(6')/aph(2") 22 izolatta (%44.9), ant(4)-la 7 izolatta (%14.3) ve aph(3')-llla 2 izolatta (%4.1) pozitif olarak saptanmıştır. Çalışmada, genotipik olarak mecA geni pozitif izolatların %72.7'sinde bir veya daha fazla AME gen birlikteliği gözlenmiştir.
Abstract
This study was conducted to investigate the presence of methicillin and aminoglycoside resistance encoding genes by multiplex-polymerase chain reaction (PCR) and by phenotypic methods in staphylococci isolated from inpatients' clinical specimens. The presence of aac(6')/aph(2"), aph(3')-llla and ant(4')-la genes encoding aminoglycoside modifying enzymes (AME) and mecA gene encoding methicillin resistance were genotypically investigated. A total of 19 S. aureus and 30 coagulase negative staphylococci (CNS) were tested. Thirty four (69.4%)' of the isolates were found to be resistant to oxacillin with disk diffusion test, 33 (97%) of which were found to harbour mecA gene. The correspondance between oxacillin resistance and presence of mecA gene was found to be 100% in S. aureus isolates, while it was 95.7% in CNS. Twenty two (44.9%), 7 (14.3%)and 2 (4.1%) isolates were found to harbour aac(6')/aph(2"), aph(3')-llla andant(4')-la AME genes, respectively. At least one or more AME genes were detected in 72.7% of mecA positive isolates.
