Çok ilaca dirençli Mycobacterium tuberculosis klinik izolatlarına karşı linezolidin izoniazid ve rifampisin ile kombine etkisi

Tüberküloz dünyada ciddi bir sağlık problemi olmaya devam etmektedir. Çok ilaca dirençli Mycobacterium tuberculosis suşları tüberküloz kontrolü için çok ciddi problemler oluşturduğundan yeni ve daha etkili kemoterapötik ajanlara ihtiyaç vardır. Bu çalışmada, çok ilaca dirençli 10 M.tuberculosis klinik izolatına karşı linezolidin tek başına ve izoniazid ve rifampisin ilekombine etkilerinin dama tahtası (checkerboard) yöntemiyle in vitro olarak araştırılması amaçlanmıştır. Çalışmada, %10 oleik asit-albümin-dekstroz-katalaz içeren Middlebrook 7H9 buyyon kullanılarak mikrodilüsyon yöntemi uygulanmıştır. Antibiyotik kombinasyonları, linezolid için 4-0.06 pg/ml ve izoniazid ile rifampisin için 32-0.03 pg/ml konsantrasyonlarda test edilmiştir. Sonuçlar, fraksiyonel inhibisyon konsantrasyon (FİK) indeksi hesaplanarak değerlendirilmiştir. Çalışılan tüm klinik izolatlar linezolide duyarlı (2 izolat için MİK= 0.25 pg/ml, 8 izolat için MİK= 0.5 pg/ml) bulunmuştur. Rifampisin MİK değerleri tüm izolatlarda > 32 pg/ml iken, izoniazid MİK değerleri 2 izolat için 4 pg/ml, 6 izolat için 8 pg/ml ve birer izolat için 16 pg/ml ve 32 pg/ml olarak belirlenmiştir. Çalışmamızda, yalnızca bir izolat için linezolid ve izoniazid arasında sinerjistik etki saptanmış (FİK indeksi: 0.265), linezolid ve rifampisin kombinasyonunda sinerjistik etki bulunamamıştır. Sonuçlarımız, linezolid ile izoniazid (bir izolat hariç) ve rifampisin arasında sinerji olmadığını vurgulamakla birlikte daha fazla sayıda izolatı içeren ileri in vitro ve in vivo çalışmalara gereksinim olduğu açıktır.
Abstract
Tuberculosis continues to be a serious public health problem worldwide. Since multi-drug resistant strains of Mycobacterium tuberculosis constitutes a serious problem in tuberculosis control, new and more effective chemotherapeutic agents are required. This study was aimed to investigate the in vitro effect of linezolid alone and in combination with isoniazid and rifampicin against 10 multidrug resistant M.tuber culosis isolates by using the checkerboard method. Checkerboard testing was performed by the broth microdilution method, using Middlebrook 7H9 broth with 10% oleic acid-albumin-dextrose-catalase. Antibiotic combinations were tested at concentrations of 0.06-4 |jg/ml for linezolid and 0.03-32 mg/ml for isoniazid and rifampicin. The results were evaluated according to the calculated fractional inhibitory concentration (FIC) index. All clinical isolates were found to be susceptible to linezolid; MICs of linezolid were 0.25 pg/ml for two strains and 0.5 pg/ml for the other strains. While rifampicin MIC values were > 32 Mg/ml for all the isolates, isoniazid MIC values were 4 ng/ml for two isolates, 8 pg/ml for six isolates, 16 Mg/ml for one isolate and 32 Mg/ml for one isolate. In this study, synergism was detected in only one strain between linezolid and isoniazid (FIC index: 0.265). Although synergy was observed between linezolid and isoniazid just for one strain, further larger scale in vitro and in vivo studies are necessary to evaluate the effect of different drug combinations against multidrug-resistant M.tuberculosis strains.
