Atriyal septal defektli hastalarda iki boyutlu kontrast ekokardiyografi ile sağ atriyumda görüntülenen negatif kontrast alanının şant büyüklüğü ile ilişkisi

Özet
AMAÇ: Atriyal septal defektli (ASD) hastalarda iki boyutlu kontrast eko ile görüntülenen sağ atriyumdaki negatif kontrast, soldan sağa şantın direkt olarak görüntülenmesini sağlamaktadır. Bu çalışma, negatif kontrast alanını (NKA) kullanarak ASD' li hastalarda şant büyüklüğünü değerlendirmenin mümkün olup olamay a cağına karar vermek için planlandı. GEREÇ VE YÖNTEM: Sekundum tipi ASD' li 24 hasta (16 bayan, ortalama yaş 32±18 yıl) çalışmaya alındı. Şant büyüklüğü spektral Doppler/2D eko yöntemiyle pulmoner ve aortik anuler seviyeden hesaplanan pulmoner (Qp) ve sistemik (Qs) kan akımlarının oranı (Qp/Qs) ve farkı (Qp-Qs) ile değerlendirildi. 14 hastaya ayrıca takip eden 1 ay içinde kateter uygulandı ve Fick metodu ile hesaplanan Qp ve Qs' den Qp/Qs ve Qp-Qs elde edildi. Kontrast eko çalışması bütün hastalarda transt o rasik (TTE) ve ilaveten 10 hastada transösofageal (TÖE) yaklaşımla uygulandı. Kontrast ajan olarak agreve edilmiş serum fizyolojik sıvı kullanıldı. Bir kalp siklusundaki maksimum ve ortalama NKA ile kantitatif Doppler eko yöntemiyle elde edilen Qp/Qs ve Q p -Qs arasındaki ilişkilere bakıldı. BULGULAR: TTE ile elde edilen maksimum (3.15±1.5 cm2/m2) ve ortalama (1.66±0.9 cm2/m2) NKA ile Doppler eko Qp/Qs (2.77±1.2) ve Qp-Qs (4.47±2.44 L/dak/m2 ) arasında anlamlı korelasyon tespit edildi (sırayla maksimum NKA için r=0.59 ve 0.73; ortalama NKA için r=0.65 ve 0.77; p<0.001). Maksimum ve ortalama NKA' nın ciddi derecedeki şantı (Qp/Qs?2) göstermedeki doğruluğu yüksekti (%79 ve 83). TEE ile görüntülenen maksimum NKA (2.72±1.72 cm 2/m2) TTE ile görüntülenenden (2.63±1.41 cm2 ) büyük olmamasına rağmen (p=0.29) birbiriyle benzerlik göstermemekteydi (ortalama fark 0.09±0.59 cm 2 ve SEE=0.19 cm2). Ortalama NKA (1.6±1.05 cm2/m2) ise TTE ile elde edilenden (1.34±0.77 cm2) büyüktü (p=0.04). Kantitatif Doppler ve kateter ile elde edilen Qp/Qs oranları (3.07±1.41 ve 2.95±1.34) birbirine benzerken (r=0.97, p<0.001; ortalama fark 0.12±0.36, SEE 0.09 ve p=0.07) şant akımları (4.51±2.7 L/dak/m 2 ve 3.77±2.57 L/dak/m2) arasında anlamlı fark vardı (r=0.87, p<0.001; ortalama fark 0.74±1.36 L/dak/m2, SEE 0.35 L/dak/m2 ve p=0.036). SONUÇ: Bu sonuçlarla, ASD' de sağ atriyumda görüntülenen negatif kontrast alanı ile şantın hem teşhis edilebileceği ve hem de büyüklüğü hakkında fikir sahibi olunacağı kanaatine varıldı.
Abstract
AIM: In patients with atrial septal defect (ASD), the negative contrast in right atrium visualized by two-dimensional contrast echocardiography permits a direct visualization of the left-to-right shunt. This study was planned to decide whether it is possible to quantify the left-to-right shunt by using of the negative contrast area (NCA) or not. MATERIAL AND METHODS: Twenty four patients (16 women; mean ages 32±18 years) with ostium secundum tupe ASD were enrolled in the study. The shunts were quantified by the quantitative Doppler echo method as the ratio (Qp/Qs) and difference (Qp-Qs) of pulmonary flow (Qp) and systemic flow (Qs) obtained at the pulmonary and aortic annular levels. Ten of the patients underwent cardiac catheterization in the following month and Qp/Qs and Qp-Qs were obtained by Qp and Qs calculated by Fick' s method. The contrast echo study was performed in all patients by transthoracic approach (TTE) and in 10 patients by additional transesophageal echo (TEE). The agitated saline was used as the contrast agent. The maximum and mean NCA in one cardiac cycle were related to the Qp/Qs and Qp-Qs obtained by the quantitative Doppler echo method. RESULTS: There were significant correlations between maximum (3.15±1.5 cm²/m²) or mean (1.66±0.9 cm²/m²) NCA and Qp/Qs (2.77±1.2) or Qp-Qs (4.47±2.44 liters/min/m²) obtained by the quantitative Doppler echo (r=0.59 and 0.73 for max. NCA, 0.65 and 0.77 for mean NCA; p<0.05, respectively). The accuracies of maximum and mean NCA in determining a severe shunt (Qp/Qs>2) were high (79 and 83%, respectively). There was no agreement between maximum NCAs (2.63±1.41 cm²/m² and 2.72±1.72 cm²/m², respectively) obtained by both TTE and TEE (mean difference=0.09±0.59, SEE=0.19 cm²/m² and p=0.29). The mean NCA (1.6±1.05 cm²/m²) obtained by TEE was found to be greater than that of TTE (1.34±0.77 cm²/m²) (p=0.04). There was agreement between Qp/Qs (3.07±1.41) obtained by the quantitative Doppler echo and that (2.95±1.34) obtained by catheterization (r=0.97, p<0.001; mean difference=0.12±0.36, SEE=0.09 and p=0.07). The Qp-Qs (4.51±2.7) obtained by the quantitative Doppler echo was found to be greater than that (3.77±2.57 L/min/m2) obtained by catheterization (p=0.036). CONCLUSION: It is possible both to diagnose the left-to-right shunt and identify the large shunt in ASD by using of the negative contrast area in the right atrium.
