Özet

Amacımız orta derecede hipertansiyon tanısıyla izlenen, renal yetersizlik, diabetes mellitusve obezitesi olmayan hastalarda kalp, gö: ve böbreklerde oluşan uç organ hasarı görülme sıklığınının araştırılması, pröghozu olumsuz yönde, etkilediği bilinen, sol ventrikü hipertrofîsi (LVH), hipertansif'retinopati (HRP) ve mikroalbüminüri (MAÜ) arasındaki bağlantının irdelenmesi, kullanılmakta olaı antihipertansif ilaçların bu komplikasyonların görülme sıklığı üzerine olan etkilerinin belirlenmesidir. Çalışma kapsamına 64 hipertansif, 10 sağlıklı normotansif olmak üzere 74 olgu alındı. Hastaların %12'si ortalama 2 aydır herhang bir antihipertansif ilaç kullanmazken, %26'sı son 6 aydır anjiotensin dönüşümcü enzim inhibitörü (ACE-Î), %23'ü betabloker %22'si kalsiyum antagonisti ve %16'sı diüretik almaktaydı. LVH'sini belirlemek amacıyla ekokardiyografık tetkik uygulanarak so ventrikül sistolik ve diyastolik fonksiyonları belirlendi, sol ventrikül kitlesi (LVM) ye kitle indeksi (LVMİ) hesaplandı. Tüm olgula, klinik ile laboratuar bulgularından habersiz olan bir oftalmolog tarafından görülerek fundoskopik muayeneleri yapıldı, Keith Wagener-Barker sınıflandırmasına göre HRP varlığı ve derecesi değerlendirildi. . Olguların ortalama hastalık süresi 7.5+3.5 yıl olup sistolik ve diyastolik kan basıncı ortalamaları sırasıyla 168+23 ve 96+6 mmllg i di. Hastaların %55 'inde HRP, %46 'sında L VH ve. %32 'sinde MAÜ mevcuttu. LVH (+) olan 30 olgunun 23 'ünde (%76) HRP sapta nırken, LVH (-) olan olguların 12'sinde (%35) HRP mevcuttu (Ki-kare=l 1.08; p<0.001). MAÜ saptanan 21 olgunun 16'sında (%75, LVH (+) iken, MAÜ (-) olan olguların 14'ünde (%32) LVH saptanmıştır (Ki-kare=12.14; p<0.001). Anlık sistolik ve diyastolik kaı basıncı değerleriyle HRP arasında (r=0.70), ve E/A oranı ve dv/dt arasında yakın bir korelasyon (sırasıyla r^O. 74, r=0.72 ve r=0.74 r=0.71), kontrol grubuna kıyasla E/A oranı, dt,dv/dt, LVM, LVMİ ve MAÜ değerleri ve göz gibi bulguları arasında anlamlı bir farı mevcuttu. Kan basıncı kontrol altına alınmış hastalarda, alınmamış olanlara kıyasla başta HRP olmak üzere LVH ve MAÜ dereces anlamlı ölçüde düşüktü (p<0.05). ACE-Lkullanan olgularda istatistiki anlamı olmamakla birlikte LVH, HRP ve MAÜ derecesi ve gö rülme oranı diğerlerine göre daha azdı. Sonuç olarak çok sayıdaki antihipertansif ilacın varlığına rağmen kan basıncının etkin kontrolü ve uç organ hasarı önlenmesinin ha len önemli bir problem olmakta devam ettiği, etkin bir kan basıncı kontrolü sağlanamadığı taktirde hangi grup antihipertansif Hat kullanılırsa kullanılsın uç organ hasarı görülme sıklığının arttığı, tutulan organlar arasında sıkı bir ilişkinin var olmasına rağmen, e; çok etkilenen organın göz olduğu, arter ve arteriollerin direkt olarak incelenebildiği tek doku olması nedeniyle düzenli olarak yaptı rılan fundoskopik muayenelerin esansiyel hipertansiyonun vasküler ve yapısal etkilerini izlemede önemli bilgiler verebileceği sonucu ' na varıldı.

Abstract

Target Organ Damage in Hypertension: Relationship Between Left Ventricular Hypertrophy, Hypertensivf Retinopathy and, Miçroalbuminuria To detect the indidence of 'cardiac, renal and ophthalmic target organ damage in patients with mild to moderate hypertension but without renal failure,diabetes mellitus or obesity;to evaluate the relation of left ventricular hypertrophy (LVH), hypertensive retinopathy (HRP) and microalbuminuha (MAU) which are known to effect prognosis unfavourably; to determine the effect of antihypertensive treatment on the incidence of those complications. 64 hypertensive patients (39 female, 25 male; mean age 54±10 years) and 10 healty normotensive controls (5 female, 5 male; mean age 52±11 years) were studied. Echocardiography was performed in all cases with a 3 MHz transducer (Vingmed CFM 750 or Hewlett Packard Sonos 1500). LVH was defined as left ventricular mass LVM>259 g in males, >166 g in females and LVM index (LVMI))> 131 g/m2 in males, >100 g/m2 females calculated according to the formula of American Society of Echocardiography. Two-dimensional and doppler echocardiography were performed to evaluate left ventricular systolic (ejection fraction) and diastolic function (mitral E to A ratio, deceleration time (dt), deceleration velocity/time (dv/dt). All subjects were examined by an ophthalmologist unaware of heir clinical and laboratory findings^ Existence and severity of HRP was assessed by the classification of Keith- Wagener-Barker. Mean duration of hypertension was 7.5±3,5 years. Mean systolic and diastolic blood pressure were 168+23 and 96±6 mmHg respectively. Incidence of HRP, LVH and MAU in hypertensive patients were found 55%, 46% and 32% respectively. HRP was detected in 23 of the 30 patients with LVH (76%), but only in 12 of the patients without LVH (35%);(ki-square=11.08; p<0.001). 16 of the 21 patients with MAU was LVH but in patients without MAU only 14 had LVH (32%); (ki-square=10.78; p<0.001). MAU was found in 18 of the 35 cases with Grade I-III HRP (51%), whereas in patients without HRP MAU was found in only 3 cases (ki- square=12.14; p<0.001). Systolic and diastolic blood pressure had good correlation with HRP (r=0.70) but such a relation was not observed with LVH and MAU. E/A ratio, dt, dv/dt, LVM, LVMI, MAU and fundoscopicexamination were significantly different between the hypertensive and control groups. LVM and LVMI and E/A ratio and dv/dt had strong correlations with each other (r=0.74, r=0.72 and r-0.74, ROO. 71 respectively). 12% of the patients had not been receiving any antihypertensive medications for 2 months, 26% were treated with art angiotensin converting enzyme inhibitor (ACE-I), 23% with a betablocker, 22% with a calcium channel blocker and 16% with a diuretic for six months. In patients with good responce to drug therapy incidence of complications and especially HRP were significantly lower than those patients whose blood pressure could not be controlled suficiently (p<0.05). Patients receiving ACE-Is had lower than those patients whose blood presusre could not be controlled suficiently not statistically significant. ., . &#8226; It is concluded, though to the many antihypertensive agents available, effective control of blood pressure and prevention of target or¬gan damage are still important problems in clinical practice. If sufficient control of biood pressure is not achieved incidence of target organ damage is very high without any diference between drug therapy regimens. There is a strong relation between the organs involved but as fundoscopic changes were observed more frequently than the other complications and because retinal arteries and arterioles are the only tissue which can be observed directly, regular fundoscopic examination can give important information about vascular and structural changes in essential hypertension.
