Akut miyokart infarktüsü geçiren sistemik lupus eritematozuslu bir olguda primer PTCA uygulaması

Özet
Sistemik Lupus Eritematozuslu (SLE) olgularda kardiyak tutulum sık olmakla beraber koroner arter hastalığı ve akut miyokard infarktüsü (Mİ) gelişmesi daha seyrek görülmektedir. Akut Mİ, SLE'nin fatal komplikasyonlarından biridir ve klasik MI olgularına göre daha genç yaşlarda ve daha çok kadınlarda görülmektedir. Hızlanmış ateroskleroz, koroner arterleri tutan vaskülit, trombüs ve koroner spazm patogenezde rol oynamaktadır. Koroner tromboz olguları genellikle antifosfolipid antikor sendromu ile birliktedir. Akut Mİ geçiren SLE'li olgularda primer PTCA uygulamaları nadirdir ve sonuçları çok iyi bilinmemektedir. Bu olgu sunumunda aktif SLE'si olup antifosfolipid antikor sendromu bulunmayan, akut Mİ geçiren ve yapılan koroner anjiografide koroner arter trombüsü saptanıp primer PTCA uygulanan bir olgu sunulmuş ve bu arada literatür gözden geçirilmiştir.
Abstract
Despite the frequent occurence of cardiac involvement in patients with SLE, development of coronary heart disease and myocardial infaction (MI) is rarely seen. Acute MI is one of the fatal complications of SLE and it is seen younger age group and especially in female patients comparing to classical MI cases. Accelarated atherosclerosis, vasculitis involving the coronary arteries, coronary thrombus and vasospazm may play a role in the pathogenesis. The cases of intracoronary trombus are usually related with antiphospolipid syndrome. Primary PTCA procedure is rare in cases of SLE with acute MI and results of this procedure has not been clearly established. In this case report, we present a case with active SLE and acute MI and without antiphospolipid syndome in whom coronary angiography revealed intracoronary thrombus and primary PTCA was performed.
