Akut anterior miyokard infarktüsünde sol ön inen arterdeki tıkanıklık yerinin saptanmasında EKG'nin değeri

Özet
Amaç: Akut miyokard infarktüsü (MI) hem yakın hem de uzak dönem prognozu etkileyen en önemli koroner arter hastalığıdır. Akut anterior MI'da sol ön inen arterin (LAD) tıkanma seviyesi de prognozu belirleyen parametrelerden biridir. Birinci diagonal arterin proksimalinde tıkanma görülen hastalarda, distalde görülen hastalara göre prognozun daha kötü olduğu gösterilmiştir. Gereç ve Yöntem: Bu çalışmada, ayrı ayrı etkinliği gösterilmiş olan EKG kriterlerini bir arada kullanarak LAD'deki lezyon yerini öngörmeyi amaçladık. Sol ön inen arterde diagonal 1 arter proksimalindeki lezyonları öngörmede; DII, DIII, aVF derivasyonlarında ST segment depresyonu, DI ve aVL derivasyonlarındaki ST segment elevasyonu, aVR derivasyonunda 1 mm ve üzeri ST segment elevasyonu, VI derivasyonunda 2.5 mm ve üstünde ST segment elevasyonu ve V6'daki ST segment depresyonu olup olmadığı değerlendirildi. Bulgular: DII, DIII ve aVF derivasyonlarındaki ST segment depresyonu miktarı proksimal grupta distal gruba göre anlamlıydı (sırasıyla, p=0.030, p=0.006, p=0.004). DI ve aVL derivasyonlarındaki ST segment elevasyonu proksimal LAD lezyonu olan grupta anlamlı tespit edilen bir diğer parametre idi (sırasıyla, p=0.003, p=0.002). V1'de 2.5 mm ve üzerinde ST segment elevasyonu ve V6 derivasyonundaki anlamlı ST segment depresyonu da LAD lezyon yerini tespit etmede oldukça öngörücü bulunmuşlardır (sırasıyla, p=0.003, p=0.012). Sonuç: Sonuç olarak; akut anterior miyokard infarktüsünde kabul edilmiş EKG kriterlerini kullanılarak LAD'deki tıkanıklığın yerinin tespit edilebileceğini gösterdik.
Abstract
Aim: Acute myocardial infarction is the most important coronary artery disease that affects short and long term prognosis. In acute anterior myocardial infarction the level of the occlusion at left anterior descending artery (LAD) is also a parameter affecting prognosis. It has been demonstrated that the patients have poor prognosis in which the occlusion is proximal to the first diagonal branch. Material and Method: In this trial we used the electrocardiographic (ECG) criteria for predicting the occlusion site in LAD, that were demonsrated as effective. For predicting the lesions proximal to first diagonal artery branch in LAD ST depression in DII, DIII, aVF; ST elevation in DI, aVL; 1mm or more ST elevation in aVR; 2.5 mm or more ST elevation in VI and ST depression in V6 were used. Result: In the proximal group ST depression in DII, DIII, aVF was found significant to distal group (respectively, p=0.030, p=0.006, p=0.004). ST elevation in DI and aVL was found to be significant compared to distal group (respectively, p=0.003, p=0.002). 2.5 mm or more ST elevation in VI and ST depression in V6 are found as predictors of the lesions proximal to first diagonal artery branch in LAD. Conclusion: As a result we had demostrated that the site of occlusion at LAD in acute myocardial infarction can be determined by using the first ECG.
