Candida glabrata'nın tanısında hızlı trehalaz testinin değerlendirilmesi

Özet
Candida türleri lokal enfeksiyonların yanı sıra ölümcül seyredebilen sistemik enfeksiyonlara da neden olmaktadır. Son yıllarda Candida albicans dışı türlerin, özellikle de Candida glabrata izolasyon oranının artışı ve flukonazole karşı C.glabrata suşlarının doza bağlı duyarlı ya da dirençli olabilmesi, tür tayininin önemini artırmıştır. Bu tip enfeksiyonların tedavisinde, etkenin hızlı ve doğru tanısı uygun antifungals seçiminde değer taşımaktadır. Bu çalışmanın amacı, C.glabrata suşlarının tanısında hızlı trehalaz testinin değerlendirilmesidir. Çalışmaya klinik örneklerden izole edilen toplam 173 Candida suşu alınmış ve izolatlar germ tüp testi, mısır unu-tween 80 ağardaki görünümleri ve Mast ID-CHROMagar Candida (Mast Diagnostics, ingiltere) besiyerindeki koloni rengi ve morfolojisi ile tanımlanmıştır. C.albicans dışı türlerin tanısı, API 20 CAUX (BioMerieux, Fransa) kiti ile de doğrulanmıştır. Buna göre suşların 86 (%50)'sı C.glabrata, 48 (%28)'i C.albicans, 17 (%10)'si C.krusei, 13 (%8)'ü C.tropicalis, 5 (%3)'i C.parapsilosis, 3 (%2)'ü C. kefyr, ve 1 (%1)'i C.utilis olarak belirlenmiştir. Tüm izolatlarda trehalaz enziminin varlığın araştırılması amacıyla, izolatlar %4 glukoz içeren Sabouraud dekstroz agar besiyerine pasajlanmış ve inkübasyondan sonra bir maya kolonisi 50 pl'lik, %4 trehaloz (ağırlık/hacim) içeren sitrat tampon çözeltisinde 37°C'de 3 saat bekletilerek süspansiyon haline getirilmiştir. Bu süre sonunda trehalaz enziminin trehalozu parçalaması sonucunda ortaya çıkan glukoz, ticari "idrar glukoz stripleri" (Spinreacta, İspanya) kullanılarak belirlenmiştir. Sonuç olarak, hızlı trehalaz testi ile C.glabrata suşlarının tümü pozitif, bir C.tropicalis suşu yalancı pozitif ve diğer tüm izolatlar negatif sonuç vermiştir. Çalışmada trehalaz testinin C.glabrata'yı tanımlamadaki duyarlılığı %100, özgüllüğü %98.9 olarak tespit edilmiş ve bu testin C.glabrata'nın tanısında hızlı, ucuz ve kolay uygulanabilir bir yöntem olduğu kanısına varılmıştır.
Abstract
Candida species which cause local infections, may also lead to fatal systemic infections. The increasing incidence of non-albicans Candida, especially fluconazole susceptible or resistant dose-dependent C.glabrata, increased the importance of rapid and accurate species level identification for Candida. Ra¬pid and correct identification of C.glabrata is essential for the initiation of the appropriate antifungal therapy. This study was conducted to evaluate the performance of the rapid trehalase test in the diagnosis of C.glabrata isolates. A total of 173 Candida strains isolated from various clinical specimens and identified according to germ tube test, growth on cornmeal Tween 80 agar and the colony morphologies on Mast-CHROMagar Candida medium (Mast Diagnostics, UK), were included to the study. The identification of non-albicans Candida species were also confirmed by API 20CAUX (BioMerieux, France) system. Accordingly 86 (50%) of the isolates were identified as C.glabrata, 48 (28%) C.albicans, 17 (10%) C.krusei, 13 (8%) Ctropicalis, 5 (3%) C.parapsilosis, 3 (2%) C.kefyr and 1 (1%) C.utilis. In order to detect the presence of trehalase enzyme in Candida strains, all isolates were grown on Sabouraud dextrose agar containing 4% glucose and then one yeast colony was emulsified in 50 (Ul of citrate buffer containing 4% (wt/vol) trehalose for 3 h at 37°C. Presence of glucose which emerged after the action of trehalase on trehalose, was detected by a commercial "urinary glucose detection dipstick" (Spinreacta, Spain). All C.glabrata strains yielded positive result by trehalase test. None C.glabrata isolates were found negative by trehalase test except for one strain of C.tropicalis. In this study, the trehalase test allowed identification of C.glabrata with 100% sensitivity and 98.9% specificity. It was concluded that trehalase test is a rapid, cost-effective and simple test that can be used for the accurate identification of C.glabrata.
