Özet

Prostetik kapak endokarditi, kardiyak kapak replasmanlannın nadir görülen, ancak ciddi bir komplikasy onudur. Er¬ken başlangıçtı proskteük kapak endokarditinde bildirilen görülme sıklığı %0,5-2 civarındadır ve %60-86 mortalite arasında mortalite ile seyreder. Geç Jbaşlangıçlı tipte ise mortalite %89 olarak belirlenmiştir. Homogreft kapaklar endokardite karşı yüksek rezistans gösterdiği için nativ ve prostetik kapak endökardiüerinde ük ter¬cih edilecek kapaklardır. Aortik homogreft implante edildikten hemen sonra kompetan bir kapağın anatomiye uygunluğa ile birlikte mükemmel bir hemodinamik düzelme sağlar. Bu nedenle potansiyel olarak yüksek mortalite taşıyan bu tip ol¬gularda aortik homogreft kullanımının en uygun olacağını düşünmekteyiz. (MN Kardiyoloji 1996; 3:254-256)

Abstract

Homograft Aortic Valve Replacement for Destructive Prosthetic Valve Endocarditis (Case Report) Prosthetic valve endocarditis is an uncommon, but serious complication of cardiac valve replacement. The reported incidence of prosthetic valve endocarditis ranges from 0,5% to 2% with a mortality of 60-80% among the patients with early-onset endocarditis. Late-onset prosthetic valve endocarditis is equally fulminant with a mortality of 89%. The viable homograft appears to have the greatest resistance to endocarditis, therefore it is the valve of choice for bacterial endocarditis affecting the native or prosthetic aortic valves. The aortic homograft Immediately offers excel¬lent hemodynamics of a totally competent valve and restores normal anatomy. We believe that aortic homograft valve replacement is the most suitable treatment for this potentially lethal complica¬tion. (MN Cardiology 1996; 3:254-256)
