Özet

Kliniğimizde elektif açık kalp ameliyatı uygulanan 68 hasta üzerinde postoperatif erken dönemde plazma faktör VIII aküvite değerlerini incelemek ve bunun postoperattf drenaj miktarına olan etkisini göstermek amacıyla çalışma yapıl¬mıştır. Hastaların preoperatif plazma faktör VIII aktivite değeri 9684.0+5.7 iken postoperatif 1. saatte % 40.6±4.3 olarak öl¬ çülmüştür. Bu iki değer arasındaki fark anlamlı bulunmuştur (p<0.01). Bu değerlere göre postoperattf 1. saatte plaz¬ ma faktör VIII aktivitesi 68 hastanın 44' ünde (%64.7) düşük, 24' ünde (%35.3) normal sınırlar içinde olduğu görül¬ müştür. , Plazma faktör VIII aktivitesi düşük olan hastaların postoperatif drenaj miktarları, plazma faktör VM aktivitesi nor¬mal veya düşük olupta faktör VIII replasmanı (50 I.U/kg) yapılan hastalara göre anlamlı olarak daha fazla olmuştur (p<0.05) . Açık kalp ameliyatı uygulanan hastaların postoperatif erken dönemde plazma faktör VIU akttvitesinin ölçülmesi ve düşük bulunan hastalarda faktör Vm replasmanı yapılması gerektiği inancındayız. Abstract

Plasma Factor VIII Activity After Open Heart Surgery This study has been performed among 68 patients who have underwent open heart surgery to evaluate the early postoperative plasma factor VIII activity levels and the effect of this on the postoperative bleeding amounts. Although the preoperattve plasma factor VIII activity ratio was 84.0+5.7%, it has been measured 40.6+4.3% in the postoperative first how. The difference between these two amounts was found significant (p<0.01). According to these values the plasma factor VIII activity was found low among 44 of the 68 patients (64.7%); among 24 patients (35.3%) it was found within the normal ranges. The postoperative bleeding amounts of patients who have low activity of plasma factor VIII were significantly higher than those who had low plasma factor VIII activity and had been administtred factor VIII (50 IU/kg). We believe that the plasma factor VIII activity of the patients who have underwent open heart surgery should be measured in the postoperative early period and factor VEI replacement should be performed in those who had low activity.
