Aterosklerotik kalp hastalığında endotelin düzeyi ile koroner arter tutulumu arasındaki ilişki

Özet
AMAÇ: Bu çalışmada egzersiz testi ve/veya talyum sintigrafisi pozitif olan 48 olgunun koroner anjiyografi öncesi periferik venden, koroner anjiyografi esnasında aort kökü ve koroner sinüsten alınan kan örneklerindeki endotelin düzeyleri incelendi. YÖNTEM: Koroner arterlerdeki %50'den daha fazla darlık eğer bir major koroner arter ve/veya büyük dallarında ise tek damar hastalığı (I. Grup), iki major koroner arter ve/veya büyük dallarında ise iki damar hastalığı (II. Grup), üç major koroner arter ve/veya büyük dallarında ise üç damar hastalığı (III. Grup) olarak gruplandırıldı. Koroner anjiyografisi normal olan hastalar IV. Grubu oluşturdu. BULGULAR: Aort kökünden alınan kan örneklerindeki endotelin düzeyi I. Grupta 2.10±0.61 fmol/l, II. Grupta 2.03±0.68 fmol/l, III. Grupta 2.20±0.94 fmol/l, IV. Grupta 1.52±0.56 fmol/l idi. I. Ve II. Grup ile IV. Grup arasında istatistiksel olarak bir farklılık yok iken (P 1=0.240, P2=0.113), III. Grup ile IV. Grubun karşılaştırılmasında istatistiksel olarak anlamlı bir fark vardı (P3=0.04). SONUÇ: Sonuç olarak koroner arter hastalığında daralmış damar sayısı ile endotelin düzeyi yüksekliği arasındaki ilişkiyi saptamada aort kökünden alınan kan örneklerindeki endotelin düzeyinin koroner sinüsten ve periferik venden alınan kan örneklerindeki endotelin düzeyine göre daha anlamlı olduğu düşüncesindeyiz.
Abstract
AIMS: In this study, endothelin levels in plasmas obtained from the aortic root and coronary sinus during angiography and endothelin levels in the peripheral venous blood before angiography in patients with positive exercise test and/or thallium sintigraphy were analysed. METHODS: Greater than 50% reduction in vessel diameter in one major coronary artery and/or in its major branches was called as single vessel disease (Group I). Similarly, if two or three major coronary artery and/or its branches showed greater than 50% reduction in diameter these were called as two or three vessel disease (Group II and Group III respectively). The patients with normal angiographic findings were included in group IV. FINDINGS: Plasma endothelin concentrations in samples obtained from the aortic root were 2. Wzt 0.61 fmol/l in group I. 2.03±0.68 fmol/l in group 11. 2.20±0.94 fmol/l in group III and 1.52±0.56 fmol/l in group IV. With respect to the endothelin concentration there was a statistically significant difference between the group III and IV (P3=0.04), while no difference was observed between the groups I. II and IV (P1=0.240, P2=0.113). RESULTS: In conclusion, the results of our study suggest that in determining the relationship between the number of the narrowed vessels and the elevated levels of endothelin in coronary artery disease, the level of endothelin in samples from the aorta root provide more meaningful information as opposed to the measurements obtained using samples from coronary sinus or peripheral blood.
