Özet

Mitral darlıkh bir hastada , balon mitral kommisürotomi sırasında, transseptal kateterizasyonu takiben aortaya girildiği ve sol ana koroner arterde diseksiyonun varlığı gözlendi. Hasta acilen operasyona alındı. Septal defekt tamir edildi. Aort duvarında küçük bir perforasyon ve etrafında bunu çevreleyen hematomun bulunduğu izlendi. Bu bölge sütüre edildi. Sol ana koroner arterde net bir perforasyon bölgesi tespit edilmedi. Mitral kapak replasmam yapılan olgunun daha sonraki günlerde tekrarlanan koroner anjiyografısinde diseksiyon ile uyumlu bulguya rastlanmadı ve taburcu edildi.

Abstract

Perforation of aorta and dissection of left main coronary artery occurred during balloon mitral commissurotomy, following transseptal catheterization in a patient with mitral stenosis. The patient was taken to operation immediately. The septa! defect was repaired successfully and mitral valve replacement was performed. The perforation of aortic wall was also repaired successfully. Any perforation place was could not find at the coronary arteries. No sign of the dissection was detected at the control coronary angiography and the patient had been discharged.
