Preoperatif kombine digitalis ve atenolol tedavisinin postoperatif atriyal fibrilasyon üzerine etkisi

Özet
AMAÇ: Koroner arter bypass cerrahisinden sonra en sık rastlanan aritmiler atriyal fibrilasyondur. Patofizyolojisi çok açık olmamakla birlikte önlenmesi ve tedavisi de suboptimaldir. Digitalis ve βblokerler tedavide sık kullanılan ilaçlardır. Bu çalışma. preoperatif başlanan digitalis ve atenolol kombinasyonunun, bu ilaçların preoperatif tek başlarına kullanımlarına göre postoperatif atriyal fibrilasyon (AF) insidansına olan etkilerinin koroner arter cerrahisini izleyen 10 gün içinde karşılaştırılması için düzenlenmiştir. GEREÇ VE YÖNTEM: Demografik özellikleri benzeşen 80 hasta randomize olarak, preoperatif kombine tedavi alan grup (grup I, n=20): preoperatif digitalis başlanan grup (grup II, n=20); preoperatif atenolol başlanan grup (grup III, n=20) ve kontrol grubu (grup IV, n=20) olarak 4 gruba ayrılmışlardır. BULGULAR: Postoperatif AF insidansı grup IV'de % 35, grup III'de % 20, grup II'de % 25 olarak görülürken (p> 0.05) , grup I 'de her üç gruba göre anlamlı olarak (%5, p<0.05) düşük bulunmuştur. SONUÇ: Atenolol ve digitalisin koroner arter cerrahisini izleyen atriyal fıbrilasyon insidansını azaltmada profilaktik preoperatif kombine kullanımı etkili bir yöntem olarak kabul edilmiştir.
Abstract
AIM: The most frequent arrythmia after coronary artery bypass surgery is atrial fibrillation. Not only the pathophysiology is unclear, hut also the treatment of this type arrythmia is subobtimal. Digitalis and βbloker, agents are preferred drugs for the treatment. This study was designed to compare the effects of the preoperatively started digitalis and atenolol combination with these drugs separately used preoperatively on the incidence of atrial fibrilation that occurs within the 7 days following coronary artery bypass surgery. MATERIAL AND METHOD: Eighty patients similar for the demographic properties were randomly grouped as: Group I: preoperativly takes combined drug therapy (n=20); Group II preoperatively takes digitalis (n=20); Group III preoperatively takes atenolol: Group IV control group (n=20). RESULTS: Postoperative atrial fibrilation incidence was 35 %, 25 %, 20 % in groups IV. III and II respectively: but was 5 % in group I and this was statistically lower compared to other groups (p<0.05). CONCLUSION: The combined use of atenolol and digitalis preoperatively was considered as an efficient method for lowering the incidence of atrial fibrillation following coronary artery bypass surgery.
