Aort darlık ciddiyetinin kontrast Doppler ekokardiyografi ile değerlendirilmesi

Özet
AMAÇ: Çalışma, aort stenozlu (AS) hastalarda darlık ciddiyetinin belirlenmesine, transpulmoner kontrast Doppler ekokardiyografinin katkısını belirlemek ve konvansiyonel Doppler yöntemi ile karşılaştırmak amacı ile planlandı. GEREÇ VE YÖNTEM: Çalışma grubu, AS ön tanısı ile ekokardiyografi laboratuarımıza gönderilen 23 hasta arasından kateter endikasyonu konulan 15 hastadan oluşturuldu (6 bayan, 9 erkek; yaş, 56±9 /yıl). Hastalara, apikal 5 boşluk pencerede CW Doppler traseleri kaydedilirken; sağ antekübital ven yolu ile 300 mg/dl SHU-580A süspansiyonundan 10 ml (bolus) enjekte edildi. Transaortik basınç gradiyenti, CW Doppler ve kateterizasyon yöntemleri ile hesaplandı. BULGULAR: Doppler akımı kayıtları, kontrast öncesinde 9 olguda net alınırken (akım derecesi 4); 6 olguda pik akım hızının ölçümüne uygun netlikte görüntü (akım derecesi 0.05). Kateter gradiyenti, kontrast öncesi gradiyent ile orta derecede (r:0.65, p<0.01) kontrast sonrası gradiyent ile ise daha güçlü bağıntı tespit edildi (r:0.88, p<0.001). SONUÇ: 1) İntravenöz SHU 580A enjeksiyonu tüm hastaların spektral CWDoppler traselerinde transaortik akım dalgalarını belirginleştirir, 2) CW Doppler trasesi net görüntülenemeyen hastalarda (Sinyal akım derecesi <3) transaortik pik akım hızı olduğundan daha düşük ölçülebilinir, 3) Bu tür hastalarda SHU 580A enjeksiyonu sinyal yoğunluğunu arttırır ve CW Doppler trasesindeki belirginleşme yaparak transaortik basınç gradiyentinin noninvazif daha güvenilir hesaplanmasını sağlar.
Abstract
This study was planned to evaluate the role of contrast Doppler echo in patients with aortic stenosis (AS) and to compare it with conventional echocardiographic methods in patients with insufficient conventional Doppler echo results. MATERIAL AND METHODS: The study group was composed of 15 patients who were selected from 23 consecuetive patients referred to our echocardiography laboratory with diagnosis of AS as they had indication for cathaterisation. While CW Doppler tracings from apical five chamber window were being obtained, 10 mlts. of Levovist (300 mg/dlt). was given through right antecubital vein way in all patients. The means oftrans-valvular gradients obtained by conventional and contrast Doppler studies and catheterisation were compared. RESULTS: Doppler tracings were obtained properly in 9 cases (signal degree 4) before contrast injection whereas Doppler tracings were not appropriate in 6 cases (signal degree <3). All cases (15 pts) have shown significant improvement in the quality of CW Doppler tracings (signal degree 4). in the patients with insufficient Doppler quality (6) maximal gradients before contrast, after contrast and catheterisation gradients were measured as 71 ± 13, 106 ± 11.3, 101±9.2 mmHg (p<0.001). The same values for the patients having sufficient Doppler tracings before contrast were as follows 95.3 ±13.2, 101.2 ±8.4 ve 98 ± 7.8 mmHg p>0.05). There was a mild degree relationship (r:0.65 p<0.01) between catheterisation and before-contrast Doppler gradients and strong relationship between cathaterisation and after-contrast Doppler gradients (r:0.88, p<0.001). CONCLUSION: 1) İV SHU 5 80A injection enhances the quality of spectral Doppler tracings of transaortic flow in all patients 2) Transaortic peak flow velocities were underestimated in patients having insufficient CW Doppler tracings (signal degree < 3) 3) injection of SHU 580A increases signal density in such patients and enables a more reliable estimation of non-invasive transaortic pressure gradient measurement
