Açıklanamayan senkop olgularının değerlendirilmesinde dil-altı düşük doz izosorbid dinitrat ile yapılan "Head Up" tilt masası testi: İsoproterenol infüzyonu ile karşılaştırma

Özet
AMAÇ: Bu çalışmada; açıklanamayan senkop atakları olan olguların değerlendirilmesinde provokasyon ajanı olarak düşük doz dil-altı izosorbid dinitrat (İSDN) kullanılan "head up" tilt masası testinin (HUTT) (HUTT+İSDN) değerini araştırmak ve bu testlerin sonuçlarını izoproterenol (İzop) ile yapılan HUTT (HUTT+İzop) sonuçlarıyla karşılaştırmak amaçlanmıştır. Senkopları diğer standart yöntemlerle açıklanamayan 43 hasta (21 kadın, ort yaş: 45.37±18.1 yıl) ile sağlıklı olduğu bilinen ve hiç senkopu olmayan 18 kontrol bireyi (8 kadın, ort yaş: 45.81±12.3 yıl) çalışma grubunu oluşturdu. Olgular 20-30 dk lık bir sırtüstü dinlenme döneminin ardından, 30 dk boyunca ya da senkop/ciddi presenkop gelişinceye değin pasif olarak 80 derece eğimle kaldırıldılar (Pasif evre). Senkop ve ciddi presenkop pozitif yanıt olarak kabul edildi. Test negatif ise, olgular aynı tilt açısındayken 2,5 mg dil-altı İSDN verildi ve 15 dk daha gözlendi (İSDN evresi). İlk 25 hastaya (10 kadın, ort yaş:46.24±17.7 yıl) ortalama 3 (1-4) hafta sonra tekrar HUTT yapıldı. Bu kez test pasif evre sonunda negatif ise hastalar sırtüstü duruma getirilip İzop infüzyonu başlandı (1 ve 3 mikrogram/dk) ve yeterli kalp hızı artışı sağlandıktan sonra tekrar 80° ye getirilip 10 dk ya da pozitif yanıt gelişinceye değin gözlendi (İzop evresi). BULGULAR: Pasif evrede 43 hastadan 10'unda (%23) HUTT pozitifken kontrol bireylerinin hiçbirinde pozitif yanıt gözlenmedi. HUTTnin İSDN evresinde ise kalan 33 hastadan 14'ünde (%42), 18 kontrol olgusunun ise 2'inde (%11) pozitif yanıt gelişti ve böylece toplam pozitiflik %56 ya yükseldi. Hafif başağrısı dışında anlamlı yan etki gözlenmedi (8 hasta-%24 - ile 4 kontrol bireyinde-%22-). HUTT +İSDN'nin pozitiflik oranı (duyarlılığı) %56 özgüllüğü ise %89 olarak hesaplandı. Hem İSDN hem de İzop ile test edilen olgulardan ISDN protokolunun pasif evresinde 5 (%20), ISDN evresinde 8 (%40) olmak üzere toplam 13 olguda (%52) HUTT + oldu. Aynı olgular İzop protokoluyla test edildiğinde pasif evrede 5 (%20) İzop evresinde de 9 (%45) olmak üzere toplam 14 olguda (%56) pozitif sonuç elde edildi. Gelişen senkop tipleri ve negatif sonuçlar açısından HUTT+İSDN ile HUTT+İzop arasında tutarlılık yüzdesi % 78.9 bulundu. HUTT +ISDN yapılırken 25 olgudan 4 ünde hafif başağrısı görülürken HUTT+İzop yapılırken çarpıntı (2), bulantı (1) ve göğüs sıkıntı (1) olmak üzere toplam 4 olguda yan etki gözlendi. SONUÇ: Düşük doz dilaltı İSDN ile provokasyon yapıldığında HUTT nin özgüllüğü oldukça yüksek kalırken toplam pozitiflik oranının belirgin olarak arttığı, HUTT+İSDN ile HUTT+İzop'un sonuçlarının ve gözlenen senkop tiplerinin oldukça benzer olduğu, HUTT+İSDN'nin HUTT+İzop'e göre daha basit ve tolere edilebilir bir yöntem olduğu ve testin pasif evresi negatif olan olgularda yaygın olarak kullanılmasının önerilebileceği düşünülmüştür.
Abstract
AIM: This study aimed to investigate the value of head up tilt table testing (HUTT) with low dose isosorbid dinitrate (ISDN) as a provocative agent in the evaluation of patients (pts) with unexplained syncope and to compare the results of HUTT+ISDN and HUTT+ Isoproterenol (Isop). Study group consisted of 43 pts with unexplained syncope (21 female, mean age 45.37±18.1 years). 18 healthy individuals without syncope were the control group (8 female, mean age: 45.81±12.3 years). Following a supine period of 20-30 mins, all pts and controls were tilted (80° tilt angle) for 30 minutes or until positive (+) response occured (passive period). If the result was negative (neg) 2.5 mg sublingual (SL) ISDN was administered at the same tilt angle and pts were observed for 15 mins more (ISDN period). First 25 of the pts (10 female, mean age:46.24±17.7 years) were tested again after a mean period of 3 (1-4) weeks. This time, if the result was neg after the passive period of 45 mins at a tilt angle of 80°, Isop was administe red (1 and max 3 microgram/min) when the patients were taken to supine position. Then, they were tilted again (80°) for 10 mins or until syncope occured (Isop period). RESULTS: In passive period, test was + in 10 pts (23%) with syncope but none in control. Of 33 pts with neg test and 18 control cases, syncope observed in 14 (42%) and 2 (11%), respectively during ISDN period. Total positivity increased to 56%. No advers effect was observed other than mild headache (8 -24%- in pts and 4 -22%- in control group). Positivity rate (sensitivity) and specificity of HUTT with low dose ISDN were calculated to be 56 % and 89 %, respectively. Among the patients (n=25) tested with both protocols, a total of 13 cases (52%) had positive results with HUTT+ISDN protocol (5 in passive period -20%- and 8 in ISDN period -40%-). 14 cases had + results (56%) when they were tested with HUTT+Isop protocol (5 in passive period -20%- and 9 in Isop period -45%-). Agreement rate was 78.9% regarding the types of syncope and the negative response during HUTT with ISDN and Isop. Mild headache was observed in 4 pts (20%) during HUTT+ISDN whereas 4 (20%) adverse events including palpitation (2), nausea (1) and chest discomfort (1) were observed during HUTT+Isop. CONCLUSION: Total positivity rate of HUTT significantly increased with low dose SL-ISDN remaining to have a high specificity, results of HUTT+ ISDN and HUTT+Isop seemed to be comparable and the response types in both tests were highly similar. This method was considered to be relatively simple and tolerable comparing with HUTT + Isop so that it might be suggested when the passive period of HUTT is negative.
