Özet

Sol atriyumda bulunan kitlelerin ekokardiyografik olarak tanımlanmasında, kitlenin şekli, görünümü, tutunma yeri ve bu kitlenin sol atriyal apendikste (SAA) olup olmaması önemlidir. Miksomalar en sık görülen primer kalp tümörleri olup, sıklıkla sol atriyumdan köken alır ve fossa ovalise tutunurlar. Özellikle atriyal fibrilasyon, spontan eko kontrast, mitral darlığı, prostetik mitral kapak ve düşük kardiyak debi varlığında SAA kitlelerinin çoğunluğunun trombüs olduğu bildirilmektedir. Sol atriyal apendikste trombüs dışı kitle olasılığı son derece nadirdir. Bu yazıda SAA'e tutunan üç adet miksoması bulunan, paroksismal atriyal fibrilasyon ve senkop ile gelen 70 yaşında bir kadın olgu sunulmuştur.

Abstract

Echocardiographic characterization of cardiac masses involving the left atrium are defined by shape and appearance, site of attachment, and presence or absence in the left atrial appendage (LAA). Myxomas are the most common primary cardiac tumors, and usually occur in the left atrium, the usual site of attachment is in the area of the fossa ovalis. Most reports use location in the LAA as strong supportive evidence of thrombus, especially if associated with atrial fibrillation, spontaneous echo contrast, stenotic mitral valve, prosthetic mitral valve, and low cardiac output state. Other masses in the LAA are considered extremely rare. In this report, we desciribed a 70-year old woman who had tree myxomas in the LAA and presented paroxismal atrial fibrillation and syncope.
