Hipertrofik kardiyomiyopatili hastalarda anjiyotensin dönüştürücü enzim geni I/D polimorfizmi ile QT dispersiyonu arasındaki ilişki

Özet
Amaç: Hipertrofik kardiyomiyopati (HKMP) düzensiz miyokart yapısı ile karakterize olup ventriküler aritmi ve ani ölüme yol açabilmektedir. Anjiyotensin dönüştürücü enzim (ADE) DD genotipinin miyokart kollajen içeriğinde artışla ilişkili olduğu ileri sürülmektedir. Bu çalışmada, HKMP’li hastalarda ve sağlıklı kontrollerde, farklı ACE genotiplerinde, ventriküler repolarizasyonda bölgesel farklılıkları yansıtan QT dispersiyonu (QTd) incelenmiştir. Gereç ve yöntem: HKMP’li 63 hasta ve 20 sağlıklı kontrol çalışmaya dahil edildi. QT parametreleri 50 mm/s hızda çekilen 12 derivasyonlu EKG’de ölçüldü. QT mesafeleri Bazett formülü ile kalp hızına göre düzeltildi. Genotipler PCR ile saptandı. Numerik değişkenler medyan (çeyreklerarası aralık) olarak ifade edildi. Bulgular: Medyan yaş HKMP ve kontrol gruplarında benzer idi [sırasıyla 60 (23,0) ve 53,5 (12,8); p=0,34]. HKMP grubunda QTd ve düzeltilmiş QTd (QTcd), kontrol grubuna kıyasla anlamlı derecede fazla idi [sırasıyla QTd: 20,0 (20,0) ve 20,0 (10,0) ms, p=0,024; QTcd: 30,0 (20,0) ve 20,0 (10,0) ms, p=0,012]. Her iki grupta da genotip frekansları benzer idi (p=0,42). Kontrol grubunda QTd ve QTcd üç ADE genotipi arasında farklı olmamasına rağmen, HKMP grubunda DD genotipli hastalarda diğer genotiplere kıyasla daha fazla bulundu (QTd [II – ID - DD]: 20,0 (0) - 20,0 (20,0) - 40,0 (10,0)ms, p<0,001; QTcd [II - ID - DD]: 20,0 (7,5) – 20,0 (15,0) – 40,0 (10,0) ms, p<0,001). Sonuç: QTd ve QTcd özellikle DD genotipine sahip HKMP’li hastalarda artmıştır.
Abstract
Aim: Hypertrophic cardiomyopathy (HCM) is characterized by disorganized myocardial architecture, and may cause ventricular arrhythmias and sudden death. The ACE DD genotype has been proposed to be associated with increased myocardial collagen content. In this study we evaluated QT dispersion (QTd), which reflects regional differences in ventricular repolarization, in patients with HCMand in controls across the three ACE genotypes. Material and Methods: Sixty-three patients with HCM and 20 healthy subjects were included into the study. QT parameters were measured from 12-lead electrocardiograms recorded at 50 mm/sec speed. QT intervals were corrected for heart rate using Bazett’s formula. Genotypes were determined by using PCR. Continuous variables were expressed as median (interquartile range). Results: Median ages were similar in HCM and control groups (60.0 (23.0) vs 53.5 (12.8), respectively; p=0.34). QTd and corrected QTd (QTcd) were significantly greater in HCM group compared to the controls (QTd: 20.0 (20.0) vs 20.0 (10.0) ms, p=0.024; QTcd: 30.0 (20.0) vs 20.0 (10.0) ms, p=0.012; respectively). Frequencies of each genotypes were similar in both groups (p=0.42). Although QTd and QTcd did not differ across three genotypes in the controls, they were significantly greater in patients with DD genotype compared to other genotypes in HCM group (QTd [II vs ID vs DD]: 20.0 (0) vs 20.0 (20.0) vs 40.0 (10.0) ms, p<0.001; QTcd [II vs ID vs DD]: 20.0 (7.5) vs 20.0 (15.0) vs 40.0 (10.0) ms, p<0.001). Conclusion: QTd and QTcd are increased in patients with HCM, especially in those with DD genotype.
