Atriyal Septal Defektlere Genel Yaklaşım ve Perkütan Yolla Kapatma İşlemleri: Tek Merkez Deneyimlerimiz

Özet

Amaç: Merkezimizde yapılmış olan perkütan yolla atriyal septal defekt kapatılması işlemlerinin başarısı, perioperatif komplikasyonları ve takip sonuçlarının değerlendirilmesi amaçlandı. Gereç ve Yöntem: Çalışmaya Ocak 2016 - Mart 2019 arasında merkezimize başvuran 52 atriyal septal defekt tanılı hasta retrospektif olarak dahil edildi. Hastaların 43 tanesi sekundum tip atriyal septal defekt, 4 tanesi primum tip atriyal septal defekt, 5 tanesi sinüs venosus tip atriyal septal defekt idi. Tüm hastalara işlemden 24 saat sonra ve 1. ayda rutin ekokardiyografik değerlendirme yapıldı. Bulgular: Perkütan kapatma yapılan hastalarda ortalama defekt çapı 20,8±5,4 mm idi. Perkütan işlem yapılan hastalarda, başarılı cihaz implantasyon oranı %95,3 (41/43 hasta) idi. İşlemin başarısız olduğu 2 hastada esas neden defekt çapının büyük olması idi. Kullanılan cihazların ortalama çapı 25,1±6,1 mm idi. 3 hastada işlem sırasında cihaz embolizasyonu görüldü. Cihaz embolizasyonu olan 3 hastada da ikinci bir deneme ile başarılı perkütan kapama uygulanabildi. İşlem sonrası ilk 24 saatlik takipte 1 hastada perikardiyal effüzyon, 2 hastada ise atriyal fibrilasyon gelişti. Sonuç: Perkütan yolla atriyal septal defekt kapatma işlemi uygun anatomiye sahip sekundum tip atriyal septal defekt hastalarında güvenli ve etkin bir tedavi yöntemidir. Ancak işlem sırasında ve sonrasında bazı komplikasyonların gelişebileceği akılda tutulmalıdır.

Abstract

Objective: The aim of this study was to evaluate the success of atrial septal defect closure procedures, perioperative complications and follow up results in our center. Material and Method: A total of 52 patients with atrial septal defect who admitted to our center between January 2016 and March 2019 were retrospectively included in this study. 43 had secundum atrial septal defect, 4 had primum atrial septal defect, and 5 had sinus venosus type atrial septal defect. All patients underwent echocardiographic evaluation 24 hours and 1 month after procedure. Results: The mean defect diameter in patients undergoing percutaneous closure was 20.8±5.4 mm. The rate of successful device implantation was 95.3% (41/43 patients). The main reason for unsuccessful procedure was the large diameter of defect size. The mean diameter of the devices used was 25.1±6.1 mm. Three patients had device embolization during the procedure. Successful percutaneous closure could be performed with a second trial in these patients. In the 24-hour follow-up, one patient had pericardial effusion and 2 patients developed atrial fibrillation. Conclusion: Percutaneous atrial septal defect closure is a safe and effective treatment for secundum atrial septal defect patients with appropriate anatomy. However, it should be kept in mind that some complications may develop during the procedure.
