Özet
AMAÇ: Sekonder kataraktlarda uygulanan Nd-YAG laser arka kapsülotomi sonuçlarının ve komplikasyonlarının değerlendirilmesi YÖNTEM: Nd-YAG laser arka kapsülotomi yapılan ve en az 1 ay (ortalama 12.3 ± 16.3ay) izlenen 71 afak veya psödofak olgunun 78 gözü çalışma kapsamına alındı. Girişim sonrası olası göz içi basıncı (GİB) yükselmesini engellemek için bütün olgulara %0.5 timolol maleat damlatıldı. Olgular kontrollere çağrılarak görme keskinliği ve komplikasyonlar değerlendirildi. Snellen eşelinde 1 sıra ve üzerinde artış, görme artışı olarak kabul edildi. BULGULAR: Kapsülotomi, katarakt cerrahisi sonrası en erken 2 ay, en geç 72 ay sonra (ort:29.5 ± 15.9 ay) yapıldı. %96.2 olguda vizyon artışı sağlandı. %0.5 timolol maleatla profilaksiye rağmen 1. saatte %14.1 oranında GİB artışı saptandı, 24 gözde (%30.8) çeşitli komplikasyonlar görüldü. SONUÇ: Nd-YAG laser arka kapsülotomi sonrası yüksek oranlarda görme artışı sağlanabilmektedir. En sık görülen komplikasyon olan GİB yükselmesi, timolol maleatla profilaksiye rağmen, erken dönemde başka bazı komplikasyonlarla birlikte yine de önemli oranlarda görülebilmektedir.
Abstract
PURPOSE: To evaluate the results and complications of Nd-YAG laser posterior capsulotomy in secondary cataracts METHOD: 78 eyes of 71 aphakic or pseudophakic cases on whom Nd-YAG laser capsulotomy was performed and followed for at least 1 month (mean 12.3 ± 16.3 months) were included in the study 0.5% timolol maleat drops were used in all cases following capsulotomy to prevent possible intraocular pressure (IOP) rise. Visual acuity and complications were evaluated in the controls. An increase of 1 or more lines in the Snellen chart was accepted as an increase in visual acuity. RESULTS: The capsulotomy was performed earliest at 2 months, latest at 72 months (mean 29.5 ± 15.9 months) after cataract surgery. An increase in visual acuity was achieved in 96.2% of the eyes. Despite prophylaxy with timolol maleate, the IOP increased in 14.1% of the eyes 1 hour after capsulotomy. Various complications were seen in 24 eyes (%30.8). CONCLUSION: Although a high rate of visual acuity increase can be achieved following Nd-YAG laser posterior capsulotomy, IOP rise which is the most common complication can be seen in the early postoperative period despite prophylaxy with 0.5% timolol maleate.
