Özet
AMAÇ: Anizometropinin binokülarite üzerine etkisini ve anizometropi olgularında füzyon ile stereopsis ilişkisini araştırmayı ve bu arada sensorial test sonuçlarını değerlendirmeyi amaçladık. METOD: Anizometropisi olan 25 hasta değerlendirildi, çalışmaya dahil edilme kriteri anizometropi mevcudiyeti idi. Her hastanın manifest refraksiyon ve Snellen eşelindeki düzeltilmiş görme keskinliği tespit edildi. Hastalar gözlük korreksiyonu ile binokülarite açısından 4-D prizma testi, Bagolini camları, Worth 4 nokia testi, TNO ve Titmus stereotestleri ile değerlendirildi. BULGULAR: Bütün hastalar Bagolini camları ile füzyon gösterdiler. 4-D prizma testi tüm hastaların anizometropik gözünde de pozitif olmakla birlikte, azalmış stereopsisi olan 19 hastada diğer gözün cevabından yavaştı. Worth 4 nokta testinde hastaların 15'inde füzyon cevabı pozitifti. TNO testinde stereokeskinlik 19 hastada çeşitli seviyelerde azalmıştı veya yoktu. SONUÇ: Ambliyopinin derinliğinin binokülarite üzerinde anizometropi miktarından daha etkili olduğu düşünüldü. Anizometropide bifoveal füzyonun zayıf da olsa hemen her olguda mevcut olduğu, ancak anizometropik ambliyopi ile orantılı olarak füzyonun zayıfladığı ve stereokeskinliğin azaldığı görülmüştür. Stereokeskinlik füzyon gücü ile ilişkilidir ve Random-dot stereotestler önemli derecede anizometropik ambliyopisi olanların normallerden ayırt edilmesinde güvenilirdir. Füzyonun tek test ile değerlendirilmesinin eksik veya yanlış bilgi verebileceği tespit edilmiştir.
Abstract
PURPOSE: We intended to research the effect of anisometropia on binocularity and relationship between stereopsis and fusion in cases of anisometropia. At the same time, we wanted to interpret the results of the sensorial tests. METHOD: Twenty-five patients with anisometropia were studied. Inclusion criteria was the existence of the anisometropia. The patients, corrected with spectacles, were evaluated by the Bagolini glasses, 4-D prism test, Worth 4-dot test, TNO and Titmus stereotests. RESULTS: All the patients indicated fusion by the Bagolini glasses. Although the 4-D prism test was positive in anisometropic eye of all 25 patients, it was slower than the other eye's response in 19 of anisometropic patients. On the Worth 4-dot test, fusion response was positive in 15 of the patients. On the TNO test, stereoacuity levels were reduced or absent in 19 of the patients. CONCLUSIONS: It is suggested that the depth of amblyopia is more effective than the amount of anisometropia in causing a detonation in binocularity. Even if fusion is weak, almost all anisometropic patients have bifoveal fusion. Fusion becomes weak and stereoacuity decreases in proportion to the anisometropic amblyopia. Stereoacuity is related to the strength of fusion, and Random-dot stereotests are reliable for discriminating subjects with significant anisometropic amblyopia from normals. The evaluation of fusion by only one test may cause obtaining incomplete or wrong information.
