Nonarteritik anterior iskemik optik nöropatide levodopa tedavisi sonuçlarımız

Özet
AMAÇ : Bu çalışmada, nonarteritik anterior iskemik optik nöropati (NAİON) tanısı ile izlenen vakalarda levodopa tedavisi ve sonuçları değerlendirildi. GEREÇ VE YÖNTEM : Çalışma kapsamına yaş ortalaması 64.7 ± 12.6 yıl olan NAİON tanısı ile ortalama 3.8 ± 1.4 yıldır izlenmekte olan 25 olgunun 27 gözü alındı. Çalışmada 24 hafta süre ile, 13 olguda levodopa, 12 olguda ise plasebo kullanıldı. Hastalarda 24. haftadan sonra ilaç kesilerek izleme devam edildi. Hastalar düzeltilmiş en iyi görme keskinlikleri, Ishihara renkli görme testi, Humprey 30-2 santral eşik görme alanı ile ortalama 32.2 ± 1.6 hafta izlendi. BULGULAR : İzlemde levodopa grubundaki 14 gözden sadece beş gözde en iyi düzeltilmiş görme keskinliğinde Snellen eşelinde iki ile beş sıralık artış saptanırken, renkli görme ile görme alanında anlamlı bir değişiklik saptanmadı. İlacın kesilmesini takiben altı hafta içinde artmış olan görme seviyelerinin tedavi öncesi değerlere gerilediği görüldü. SONUÇ : Levodopa tedavisi NAİON'de bazı olgularda görme keskinliğinde artışa neden olsa da etki ilaç kesildikten sonra gerilemektedir.
Abstract
PURPOSE: In this study, the results of levodopa therapy in patients with non-arteritic ischemic optic neuropathy (NAION) were evaluated. MATERIAL AND METHODS: Twenty-seven eyes of 25 NAION patients with the mean age 64.7 ± 12.6 years, and mean follow-up 3.8 ± 1.4 years were included in the study. In 13 cases levodopa, and in 12 cases placebo was given for 24 weeks. After 24 weeks, the therapy was ceased and follow-up continued. The patients were followed-up with a mean of 32.2 ± 1.6 weeks with best corrected visual acuity (BCVA), Ishihara color plates, and Humprey 30-2 central threshold visual field analysis. FINDINGS: In the follow-up, in levodopa group, BCVA increased two to five Snellen lines in 5 eyes whereas the difference in color vision and visual field was non-significant. Following the cessation of levodopa therapy, in six weeks increased visual acuities regressed to pretreatment levels. CONCLUSION: Although levodopa therapy yields better visual results in some cases with NAION, the effect is reversible after the cessation of therapy.
