Keratokonus olgularında femtosaniye lazerle kornea içi halka yerleştirilmesi ve cep oluşturularak kombine çapraz bağlama tedavisi

Özet
Amaç: Femtosaniye lazer kullanarak kornea içi halka (KİH) implantasyonu için tünel ve çapraz bağlama tedavisinde riboflavinin intrastromal uygulanması için cep oluşturmasını içe- ren ve ektatik korneaların tedavisinde kullanılan yeni yöntemin değerlendirilmesi. Gereç ve Yöntem: Femtec femtosaniye lazer kullanılarak önce kornea kalınlığının %70 de- rinliğinde tünel ve devamında epitelden 100 mikron derinlikte olacak şekilde bir kornea cebi oluşturuldu. KİH'ın yerleştirilmesini, intrastromal riboflavin enjeksiyonu ve Ultraviyole A ir- radiasyonu takip etti. Bu işlem sırasında kornea epiteli kazınmadı.Cerrahi işlem sonrasında tüm olgular en az 6 ay takip edildi. Bulgular: İki hastanın 3 gözüne KİH implantasyonu ve intrastromal riboflavin uygulamalı çapraz bağlama tedavisinden oluşan kombine prosedür uygulandı. Tüm gözlerde keratometri ve sferik eşdeğer verilerde anlamlı düzelme gözlendi. Tashihli görme keskinliklerinde 3-4 sıra ar- tış izlendi. Takip süresince keratokonus gözlerde progresyon görülmedi. Olgular cerrahi işlem- den 2 gün sonra tüm aktivitelerine dönebildiler. Hiçbir gözde komplikasyon gelişmedi. Sonuç: Epitelin korunması ile yapılan bu yeni kombine prosedür keratokonus gözlerin te- davisinde umut verici sonuçlar vermektedir.
Abstract
Aim: To evaluate the new approach for preparing corneal tunnel to implant intrastromal corneal ring segments and to create intrastromal pocket to apply riboflavin during crosslink- ing treatment with the femtosecond laser to treat ectatic corneas. Material and Method: With Femtec femtosecond laser first, intrastromal corneal tunnel of 70% corneal depth and then 100 microns beneath corneal epithelium pocket was created. Injection of riboflavin 0.1% and ultraviolet A irradiasyon followed intrastromal ring implan- tation. During this process, corneal epithelium was not removed. After the surgical procedure, all patients were followed up for at least 6 months. Results: Combined procedure of intrastromal ring implantation and intrastromal cross- linking treatment with riboflavin were performed to 4 eyes of 3 patients. Significant improve- ment was observed in all eyes with the reduction in keratometric and spherical equivalent val- ues. Best corrected visual acuity increased 3-4 lines. Keratoconus progression was not ob- served during follow-up. Patients were able to return to all activities 2 days after the surgical procedure. No complication was encountered in any of the eyes. Conclusion: This new combined procedure with the protection of the epithelium gives promising results for the treatments of ectatic eyes.
