Keratokonus değerlendirmesinde elevasyon ve plasido disk sistemli topografi cihazlarının videokeratografik indeksler kullanılarak karşılaştırılması

Özet
AMAÇ: Keratokonus ve benzeri durumlarda farklı korneal topografi sistemlerinden alınan verileri nümerik indeksler kulanarak karşılaştırmak. YÖNTEM: 7 astigmatizma, 10 keratokonüs şüphesi ve 24 keratokonüsden oluşan toplam 41 gözde elevasyon ve kurvatür haritasını kombine eden PAR korneal topografi sistemi (PAR CTS) ile sadece kurvatür haritası veren plasido disk sistemli Humphrey ve Alcon cihazları birbirleriyle karşılaştırıldı. Bu karşılaştırma için, tanımlanan nümerik indekslerden simulasyon keratometri değerleri (sim K1 ve K2), "differential sector indeks" (DSİ), "opposite sector indeks" (OSİ) ve "center/surround indeks" (CSİ) kullanıldı. Ayrıca aynı göze ait elevasyon haritası ile kurvatür haritaları kalitatif olarak gerçek topografik (elevasyon) kon lokalizasyonu ile en dik kornea alanı lokalizasyonu arasındaki tutarlılık açısından değerlendirildi. BULGULAR: Kullanılan 5 nümerik indeks yönünden 3 cihazın sagital kurvatür haritalarından alınan ölçümler arasında anlamlı fark yoktu. Ancak non-keratokonüs (17 göz) ve keratokonüs (24 göz) olarak değerlendirme yapıldığında, tüm parametrelerde anlamlı fark çıktı. Özellikle PAR CTS kurvatür haritasına ait OSİ ve CSİ sonuçları ileri derecede anlamlı bulundu. PAR CTS elevasyon haritasının kalitatif değerlendirmesi ise, ilginç olarak gerçek topografik ektazi ile kurvatür haritasındaki en dik kornea alanının (kon) farklı kornea kadranlarında lokalize olabildiğini gösterdi. SONUÇ: Sistemleri farklı topografi cihazlarının kurvatür haritası verileri arasında fark olmamakla beraber elevasyon haritasının kurvatür haritasıyla beraber kullanılması erken ektazi gibi kornea şekil anormallikleri için topografiyi daha duyarlı bir gösterge yapabilir.
Abstract
PURPOSE: To compare the data which is obtained different topographic systems by using numeric indices in patients with keratocoruis and in those resemble keratoconus. METHOD: PAR corneal topography system (PAR CTS) which combines the elevation and curvature maps, and 2 placido disc based systems (Humphrey and Alcon) which yield only curvature maps were compared with each other in a group of 41 eyes which consist of 7 eyes with astigmatism, 10 eyes with keratoconus suspect, and 24 eyes with keratoconus. For this comparison, the numeric indices including simulated K1 (Sim K1) and Sim K2, "differential sector index" (DSI), "opposite sector index" (OSI) and "center/surround index" (CSI) were used. In addition, the elevation map of PAR CTS were qualitatively evaluated with the curvature maps of the same eye by means of the correlation between the localization of true topographic cone and the steepest corneal area. RESULTS: There was no statistically significant differences between the indices obtained the curvature maps of three topography. However the evaluation between non-keratoconus (17 eyes) and keratoconus (24 eyes) was statistically significant in all parameters. Especially OSI and CSI results from the curvatur maps of PAR CTS were extremely significant. The qualitative evaluation of PAR CTS elevation maps interestingly revealed that the maximum curvature (cone) in the sagittal curvature map and the true ectasia in the elevation map can be localized in different corneal quadrant. CONCLUSION: Although the curvature data of different topographic systems were not different, the combined use of elevation and curvature maps may be a more sensitive indicator of corneal shape abnormalities such as early ectasia.
