Özet

AMAÇ: Büyük üst kapak tümörlerinin eksizyonuna ikincil gelişen kapak enformasyonlarında kullanılabilecek tek basamaklı rekonstrüktif cerrahi metodları tartışılmaktadır. YÖNTEM: Tümör eksizyonları sonucu üst kapakta oluşan büyük ve tam kat kapak deformitelerin e tek seansta cerrahi uygulanmıştır. İki olguda kapağın en az yarısını tutan üst kapak tümörleri mevcuttu. Bu olgularda karşı taraftan alınan serbest otojen tarsokonjunktival greft ile arka lamella oluşturulurken, ön lamella rekonstrüksiyonu için lokal miyokutanoz flep ilerletilmesi uygulandı. Böylece miyokutanoz flep serbest tarsal greft için vasküler destek sağlamış oldu. Üst kapağında totale yakın tümörü bulunan iki olguda alt kapak rotasyon ve inversiyon tekniği uygulandı. Bu teknikte alt kapak tabanı dışta olacak şekilde tam kat insize edilerek tümüyle serbestleştirildi ve çevrilerek üst kapağa rotasyon flebi olarak sütüre edildi. Bu, yanağın dış kısmının Mustarde'nin yanak rotasyon flebi ile sağlandı. Böylece alt kapak ön lamellası da oluşturulurken arka lamella rekonstrüksiyonunda bukkal mukoza grefti kullanıldı. Bu şekilde alt ve üst göz kapağının aynı seansta rekonstrüksiyonu mümkün oldu. Rekonstrüksiyon sırasında sağlam kapağına müdahale istemeyen büyük santral defektli bir olguda, arka lamella sert damak mukoza grefti ile oluşturulup bukkal mukoza grefti ile kaplandı. Ön lamella ise lokal miyokutanoz flep ilerletme tekniği ile oluşturuldu. BULGULAR: Olgularda mevcut olan bazal hücreli kanser, yassı hücreli kanser, malign melanositik melanom ve sebase hücreli karsinom eksize edildikten sonra gelişen kapak deformasyonlarının rekonstrüksiyonu sonrası tatminkar kozmetik ve cerrahi başarı saptanmış olup ciddi komplikasyon izlenmemiştir. SONUÇ: Üst kapağın daha fazla hareket ihtiyacı, globu ve korneayı koruyucu etkisi ve rekonstrüksiyonda kullanılabilecek alt kapak tarsının kısıtlı olması tam kat deformasyonların tamirini zorlaştırmaktadır. Klasik olarak kullanılan yöntemlerin çoğu iki basamaklı cerrahi gerektirirken, uygulamış olduğumuz tek basamaklı cerrahi teknikler hastayı hem maddi hem de ikinci cerrahinin getireceği manevi güçlüklerden korumakta, cerraha ise zaman kazancı ve teknik kolaylık sağlamaktadır.

Abstract

PURPOSE: In this paper we try to subject the one stage repairment of large upper eyelid defects after tumor excision. METHOD: One stage reconstruction surgery was performed in large full thickness upper eyelid defects secondary to tumor excision. In two cases, laterally and medially localized eyelid defects were reconstucted with free tarsoconjunctival grafts and local miyocutaneous advancement flaps. In this technique free tarsoconjunctival graft from contralateral upper eyelid was used to supplement the posterior lamellar lining and anterior lamella was reconstructed with undermining and advancing local skin and orbicularis muscle. In two cases with total upper eyelid defects, we used lower lid rotation and inversion flap technique. In this procedure a full thickness laterally based lower lid rotation flap was prepared and inverted to reconstruct the upper lid defect. The lateral aspect of the lower lid is advanced medially with Mustarde's cheek rotation flap. This provided the anterior lamella of the lower lid and the posterior lamella is reconstructed with buccal mucosa graft. This procedure enables reconstruction of both upper and lower eyelids at the same sitting. In one case who did not accept a procedure involving the healthy lower eyelid, the posterior lamella was reconstructed with chondromucosal hard palate graft which was covered with buccal mucosa. Miyocutaneous advancement flap was used for the anterior lamella. RESULTS: All cases were reconstructed with physiologic and functionally satisfactory results after excision of basal cell carcinoma, squamous cell carcinoma, malign melanoma and sebaceous cell carcinoma. CONCLUSION: The greater mobility required for the upper eyelid, its role in ocular protection and limited amount of tarsal tissue available in the ipsilateral lower eyelid are difficulties in reconstructing full thickness large upper eyelid defects. Various one stage surgical techniques for the reconstruction of the upper eyelid were advantageous for both the patient and the surgeon.
