Özet

AMAÇ: Anterior etmoid hücreleri sıklıkla lakrimal kese fossasının medialinde yer alır. Bu anatomik ilişki nedeniyle eksternal dakriosistorinostomi (DSR) ameliyatında etmoid sinüse girilebilir. Bu çalışmada, idiopatik obstrüktif epiforaya yönelik primer DSR ameliyatında etmoid sinüsle karşılaşma sıklığı ve ameliyat sonuçlarına etkisi prospektif olarak araştırıldı.YÖNTEM: Osteotomi periost elevatörü ve kemik panç yardımıyla yapıldı. Üstte ortak kanalikülün çevresinde 5 mm'lik açıklık bırakacak şekilde, önde nazomaksiller sütür hattıyla sınırlı, altta nazolakrimal duktus medialini içeren bir kemik pencere oluşturuldu. Arkada perforasyon hattının daha posterior kısmından kemik alınmadı. Etmoid sinüsün varlığı, osteotomi alanında makroskopik olarak sinüse özgü anatomik özelliklerin gözlenmesiyle belirlendi. BULGULAR: Dakriosistorinostomi yapılan 46 hastanın 28' inde (% 61) osteotomi alanında etmoid sinüs ile karşılaşıldı. Başarı oranı etmoid sinüse girilen grupta 26/28 (% 93), diğer grupta 16/18 (% 89) idi; fark istatistiksel olarak anlamlı değildi (P< 0.05). Ortalama izlem süresi 7.0 aydı (4 -12 ay). SONUÇ: Eksternal DSR' de osteotomi lakrimal sütürden girilerek yapıldığında sıklıkla anterior etmoid sinüs ile karşılaşılmaktadır. Eksternal DSR' de etmoid sinüse girmenin avantajları ve riskleri gözönünde tutulduğu takdirde, ameliyat sonucu olumsuz etkilenmeyecektir.

Abstract

PURPOSE: Anterior ethmoidal sinus frequently lies medial to the lacrimal sac fossa. Because of this anatomic relationship, it is possible to enter ethmoid sinus when performing external dacryocystorhinostomy (OCR). In this study, we prospectively investigated the incidence of entry into the ethmoidal sinuses during DCR and its effects on the surgical outcome in cases with primary idiopathic obstructive epiphora. METHOD: Osteotomy was performed by using periostal elevators and bone punch. The border of the bony opening was superiorly within 5 mm of the common canaliculus, anteriorly limited with the nasomaxillar suture line and inferiorly including the medial part of the nasolacrimal duct. Posteriorly no bone was removed from posterior side of the initial perforation line. The specific features of the ethmoidal sinus exposure was macroscopically identified in this osteotomy opening. RESULTS: The entrance to the ethmoidal sinus occurred in 28 of 46 external DCR procedures (61 %). The success rate was 26/28 (93 %) in the group with ethmoidal sinus entry, and 16/18 (89 %) in the other group; this difference was not significant statistically (P<0.05). The mean follow up was 7.0 months (4-12 months). CONCLUSION: Anterior etmoidal sinus is frequently entered in external DCR in which the ostium was performed via the lacrimal suture. When being aware of the operative risks and advantages of entering an ethmoidal sinus, the surgical outcomes are not effected negatively.
