Excimer lazer fotorefraktif keratektomi ile düşük ve orta dereceli miyopi ve miyopik astigmatizma tedavisi

Özet
AMAÇ: Bu çalışmanın amacı, düşük ve orta dereceli miyopi ve miyopik astigmatizmaların tedavisinde uygulanan excimer lazer fotorefraktif keratektomi (PRK) ile elde edilen sonuçların stabilitesi ve tahmin edilebilirliği ile olası komplikasyonları incelemektir. YÖNTEM: Bu çalışmada, Mayıs 1994-Aralık 1997 yıllarında, excimer lazer PRK tedavisi görmüş 96 hastanın 190 gözü ortalama 22 aylık takip sonucu değerlendirilmiştir. Preoperatif miyopi dereceleri -2.0 dioptri (D) ile -6.0 D, astigmatizma ise -1.0 D ile -4.0 D arasında idi. Çalışmamızda, Technolas Kerakor 116 (Münih) argon florid 193 nm excimer lazer kullanıldı. Geniş saha tekniği kullanılarak 7 mm'lik optik zonla çalışılmış, tüm hastalara postoperatif standart topikal steroid tedavisi uygulanmıştır. Değerlendirme kriteri olarak postoperatif düzeltilmemiş görme keskinliği ve arzulanan emetropi yüzdesi alınmıştır. BULGULAR: Postoperatif 6 aylık stabilizasyon sonrası arzulanan emetropi, miyopili hastalarda ± 1.0 D aralığında 178 göz (%94), miyopik astigmatizmalı hastalarda ise ± 1.0 D aralığında 56 göz (%87.5), postoperatif düzeltilmemiş görme keskinliği 8/10-10/10 aralığında 133 göz (%70) olarak tespit edilmiştir. Bir hastanın iki gözünde steroid kullanımı sırasında göziçi basınç artışı izlenmiştir. SONUÇLAR: Düşük ve orta dereceli miyopi ve miyopik astigmatizmaların tedavisinde PRK, güvenilir, etkin, tahmin edilebilir ve komplikasyon oranı düşük bir cerrahi yöntemdir. Stabilizasyon postoperatif 6. ayda sağlanabilmektedir.
Abstract
PURPOSE: The purpose of this study is to investigate the stability and predictability of the results, along with the possible complications, taken by excimer laser photorefractive keratectomy (PRK) performed in the treatment of low and moderate myopia and myopic astigmatism. METHODS: In this study, the results for 190 eyes of 96 patients who underwent excimer laser PRK treatment between May 1994 and December 1997 have been evaluated with a subsequent 22-month follow-up period. The preoperative myopia degrees were ranged from -2.0 D to -6.0 D, and the astigmatism degrees from -1.0 D to -4.0 D. The Technolas Kerakor 116, (Munich) argon fluoride 193 nm excimer laser was used in our study. The operation was performed using the enlarged treatment zone technique by a 7-mm optical zone and the postoperative standard topical steroid treatment was applied to each patient. As the evaluation criteria of PRK the postoperative uncorrected visual acuity and intended emetropia percentages were taken. RESULTS: Following a postoperative six-month stabilization, the intended emetropia range was determined to be at the interval of ± 1.0 D in 178 eyes (94%) for the patients with myopia and ± 1.0 D in 56 eyes (87.5%) for the patients with myopic astigmatisma and the postoperative uncorrected visual acuity at the interval of 8/10-10/10 in 133 eyes (70%). in the two eyes of one patient an increase in intraocular pressure was observed during the steroid appliance. CONCLUSION: PRK is a safe, effective and predictable surgical method, with a low rate of complication, in the treatment of low and moderate myopia and myopic astigmatism. The stabilization can be provided postoperatively in the 6th month.
