Özet
AMAÇ: Konjonktivanın yassı hücreli karsinomunda (YHK) klinik özellikler ve tedavi sonuçlarının değerlendirilmesi. YÖNTEM: 1965-1998 yılları arasında histolojik olarak YHK tanısı almış 309 olgunun (311 göz) dosyaları geriye dönük olarak incelenmiş, yeterli izlem olmayan hastalar telefon veya mektup ile kontrole çağrılmıştır. Olgular yaş, cinsiyet, yerleşim yeri ve uygulanan tedavi açısından değerlendirilmiştir. BULGULAR: YHK tanısı konan 309 olgunun %45'i kadın, %55'i erkek, yaş ortalaması 53.46 ± 15.82 yıl idi. İlk tedavi olarak olguların %11.2'sine Total Eksizyonel Biyopsi (TEB), %4.5'ine sklerokeratoplasti, %25.4'üne TEB ve kriyoterapi, %32.4'üne TEB ve Str-90, %11,2'sine enükleasyon, %4.8'ine ekzenterasyon, %9.3'üne ekzenterasyon ve Radyoterapi tedavilerinin uygulandığı saptanmıştır. Birinci tedaviyi takiben %15.1 olguda 12.05 ± 6.39 ay sonra nüks tespit edilmiştir. İkinci tedavi olarak TEB ve Kriyoterapi, TEB ve Str-90, enükleasyon, enükleasyon ve radyoterapi, ekzenterasyon, ekzenterasyon ve radyoterapi uygulanmıştır. İkinci nüks % 15.2 olguda 9.71 ± 4.03 ay sonra saptanmıştır. Üçüncü tedavi olarak %60 olguya TEB ve Str-90, %40 olguya ekzenterasyon uygulanmıştır. Üçüncü tedaviyi takiben 1 olguda üçüncü nüks saptanmıştır. SONUÇ: Konjonktiva yassı hücreli karsinomlarının en yaygın tedavisi geniş lokal eksizyon ve nüksleri önlemeye yönelik kriyoterapi Str-90 ikincil tedavileridir. Yetersiz tedavilerde nüks sonucu enükleasyon, ekzenterasyon gibi kökten cerrahi girişimlerin gündeme gelebileceği unutulmamalıdır. Ameliyat sonrası izlem çok özellikle ilk 3 yıl çok önemlidir.
Abstract
PURPOSE: To evaluate the clinical characteristics and results of treatment in squamous cell carcinoma of the conjunctiva. MATERIAL AND METHOD: Records of 309 patients (311 eyes) with histologically proven diagnosis of SCC admitted between 1965-1998 were retrospectively evaluated and recruited with left and telephone Age, sex, treatment and demographic characteristics of the cases were compared. RESULTS: Mean age of the 309 patients with the diagnosis of SCC was 53.46 ± 15.82 yrs and of the patients 45% was female, 55% was male. 11.2% had Total Excisional Biopsy (TEB), 4.5% had sclerokeratoplasty, 25.4% had TEB and cryotherapy, 32.4% had TEB and Str-90 radiation therapy, 11.2% had enucleation, 4.8% had exanteration, 9.3% had exanteration and radiotherapy as primary treatment. Following primary treatment recurrence was seen in 15.1% of the cases after 12.05 ± 6.39 months. As secondary treatment TEB and cryotherapy, TEB and Str-90, enucleation, enucleation and radiotherapy, exenteration, exanteration and Radiotherapy were performed. Second recurrences were detected in 15.2% of the cases after 9.71 ± 4.03 months. As third treatment 60% of the patients had TEB and Str-90 and 40% had exenteration. After third treatment recurrence was seen in one case. CONCLUSION: Extensive local excision is the most preferred treatment for conjunctival SCC cases and for prevention of recurrences cryotherapy, Str-90 were applied as a secondary treatment. Due to inadequate treatment, recurrences can be seen and radical treatment modalities such as enucleation and exenteration can be necessary. Follow up visits after surgical treatment have great importance, especially for the first 3 years.
