Özet

AMAÇ: Temporal arterit (dev hücreli arterit, kranial arterit) sebebi bilinmeyen, körlük, felç ve ölüme sebep olabilen granülomatöz, iltihabi bir hastalıktır. Çalışmamızın amacı iskemik optik nöropati tanısı almış olguların içinde temporal arterit orijinli olanları tanı ve klinik açıdan incelemektir. YÖNTEM: Anterior iskemik optik nöropati (AION) tanısı ile retina birimimizde takip edilen yaşları 60-80 arasında değişen 50 hasta incelenmiş, metabolik, toksik, beslenme, infiltrasyon ve kompresyon etiyolojili olanlar ayırt edildikten sonra 8 olguda temporal arteritten şüphelenilmiştir. BULGULAR: 8 olgunun 7'sinde temporal arter biopsisinde temporal arteritis ile uyumlu histopatolojik bulgular saptanmıştır. 1 olguda biopsi negatif çıkmış ancak diğer bulgular temporal arteriti desteklediği için tanı konulmuştur. 8 olgunun 6 tanesi bilateral tutulumlu idi. Olgularımızda sedimantasyon ESR 28-80 mm/saat arasında değişmekte idi. Bu hastalara yüksek doz oral kortikosteroid başlanarak ESR kontrolü ile 6 ay 1 yıl arası doz ayarlanarak devam edilmiştir. SONUÇ: Ortalama 8 aylık takip süresince tedavi edilen olguların görme derecelerinin sabit kaldığı ve unilateral tutulumlu 2 olgunun diğer gözlerinin etkilenmediği izlenmiştir.

Abstract

AIM: Temporal arteritis which is also called giant cell arteritis is a systemic granulomatous vasculitis of unknown cause that can lead to blindness, stroke and death. The aim of our study is examining the cases diagnosed as anterior ischemic optic neuropathy (AION) and especially focusing on diagnosis and clinical features of temporal arteritis. METHODS: 50 cases of AION ages differing between 60-80 were observed between years 1993-1997 in our retina department. After exclusion of metabolic, toxic, nutritional, infiltrative and compressive etiologies, a preliminary diagnosis of temporal arteritis was made in 8 cases. RESULTS: In 7 cases the diagnosis was based on histopathological findings. 1 case was biopsy negative, but other findings were consistent with temporal arteritis. 6 of 8 cases were bilateral. Sedimentation rate was found to be between 28-80 mm/hr. These patients were put on a high dose oral corticosteroid regime and theraphy was continued for a period of 6 months to 1 year and monitored with sedimentation rate. CONCLUSION: In the mean follow up of 8 months, the visual acuity of the cases was found to be stable and in 2 cases with unilateral disease the other eyes did not show the symptoms of the disease.
