Üst temporal sklerotomi yoluyla evisserasyon ve polietilen implant uygulanımı

Özet
AMAÇ: Üst temporal ekvatoryal sklerotomi yolu ile evisserasyon uygulanarak polietilen implant yerleştirilen hastalarda başarı ve komplikasyonlar incelendi. GEREÇ VE YÖNTEM: Ocak 1996-Mart 1999 tarihleri arasında Kartal Eğitim ve Araştırma Hastanesi 1. Göz Kliniğinde kornea dokusu korunarak üst temporal sklerotomi ile evisserasyon yapılan 14 göze polietilen implant uygulanmıştır. Cerrahi endikasyonlar, implant boyları, klinik gözlem, komplikasyonlar ve aralıklı takipler kaydedilmiştir. Evisserasyon yapılan 14 gözün 8 tanesi travma (% 57.1), 4 tanesi fitizis bulbi (% 28.6), 2 tanesi ise buftalmus (% 14.3) nedeniyle ameliyat edilmiştir. BULGULAR: Takip edilen gözlerin bir tanesinde üst sklerotomi bölgesinden implantın konjonktiva altına çıktığı görülmüştür (%7). Buftalmus nedeniyle 10 aylık iken polietilen sfer uygulanan bu hastaya okülarist tarafından protezin göz küresine büyük uygulandığı saptanmıştır. Diğer 13 gözden 12'sine protez uygulanmış, hiçbirisinde komplikasyonla karşılaşılmamıştır. SONUÇLAR: Korneanın korunması ve üst ekvatoryal sklerotomi ile implantın yerleştirilmesi hem atılımı azaltması yönünden hemde protez yapılana kadar geçen sürede kozmetik kontakt lenslerin kullanılmasını sağlayarak hasta memnuniyetini arttırmaktadır
Abstract
PURPOSE: We investigated the success and complications of a surgical technique of evisceration using polyethylene orbital implant MATERIAL AND METHOD: We performed evisceration sparing corneal tissue with sclerotomy via upper temporal equatorial region in fourteen eyes between January 1996 and March 1999 in Kartal Training and Research Hospital. Patient records were reviewed for surgical indication, orbital implant size, clinical outcometcomplications and follow-up interval. The cause of evisceration was trauma in eight eyes (% 57.1), phtisis bulbi in four eyes (%28.6) and buphtalmos in two eyes (14.3%) RESULTS: During the follow-up period one of the spheres extruded via the upper equatorial sclerotomy site to the subconjonctival space (7%).Prosthesis which was implanted by the ocularist to a 10-month-old baby whose eye had been eviscerated due to buphtalmos was determined oversized and that sphere was subsequently rejected as well. CONCLUSIONS: Prosthetic implant was performed in twelve of the remaining thirteen eyes no further complications were experienced. Protection of cornea and the upper sclerotomy site and the implant enhances the patient's contentment in terms of both reducing extrusion and providing the use of cosmetic contact lenses until the prosthesis is prepared.
