Konjenital ile akkiz nistagmusların ayrılmasında elektro-nistagmografinin yeri

Özet
AMAÇ: Konjenital nistagmusu olan erişkin hastaların nistagmus paternlerini diğer santral patolojilere bağlı olarak ortaya çıkan nistagmustan ayırmada elektro-nistagmografinin yerini araştırmak. YÖNTEM: Konjenital nistagmusu olan 21, santral patolojiye bağlı nistagmusu olan 17, normal grubu oluşturan 30 kişinin göz hareketleri, klinik olarak değerlendirilmiş ve ICS MASTR kompüterize elektro-nistagmografi cihazı ile incelenmiştir. Sonuçlar yüzde oranlar şeklinde dokümente edilmiştir. BULGULAR: Konjenital nistagmus grubunda en fazla gözlenen patoloji sakkad bozukluğu olup, 18 hastada saptanmıştır. Daha sonra sırasıyla bakış bozukluğu 17 olguda, takip bozukluğu 14 olguda, iki olguda pozisyonel test ve dört olguda optokinetik test bozukluğu saptanmıştır. Santral patoloji grubunda bakış testinde iki hastada vertikal, bir hastada horizontal nistagmus saptanmıştır. Onüç hastada takip bozukluğu, 11 hastada sakkad bozukluğu, 8 hastada pozisyonel nistagmus, 4 hastada ise optokinetik cevap asimetrisi izlenmiştir. Santral patolojilere bağlı nistagmus paterni yukarı vurucu nitelik göstermiş, pozisyonel testlerde bu nitelikte bozulma ortaya çıkmıştır. Kontrol grubunu oluşturan 30 kişiden 4'iinde pozisyonel test bozukluğu dışında, elektro-nistagmografik muayene normal bulunmuştur. SONUÇ: Otomatik analizleme, diskete veri kaydı kolaylığı, göz hareketlerini ve nistagmus paternlerini kaydedebilme özellikleri ile göz hareketlerinin incelenmesinde ve analizinde elektro-nistagmografi objektif ve güvenilir bir yöntemdir.
Abstract
PURPOSE: To investigate the role of electro-nystagmography in differentiation of the patterns of congenital and other nystagmus due to central lesions. METHOD: Eye movements of 21 patients with congenital nystagmus, 17 patients with nystagmus due to central lesions and 30 normal subjects were evaluated clinically and investigated by ICS MASTR computerized electro-nystagmography. RESULTS: The most frequently observed pathology in the congenital nystagmus group was saccade disorders. This disorder was observed in 18 of 30 patients. Gaze disorders were seen in 17 cases, pursuit disorders were seen in 14, in 2 cases there were positional test disturbances and optokinetic tests were abnormal in 4 cases. In the group of central lesions, there was vertical nystagmus in two patients, horizontal nystagmus was present in 1 patient. Besides, pursuit disorders were detected in 13 patients, there were saccade disorders in 11 patients, positional nystagmus in 8 patients, and asymmetry of optokinetic response was observed in 4 patients. In case of nystagmus due to central lesions, an upbeating pattern and disturbance of this pattern in the positional tests were observed. In the control group consisting of 30 subjects, except for the 4 patients exhibiting positional test disturbances, electro-nystagmographic examination was normal. CONCLUSION: Electro-nystagmography is an objective and safe method in the analysis and investigation of eye movements because of its properties like the computerized analysing, data protection in magneto-optic discs and saving nystagmus patterns.
