Fakoemülsifikasyonda öğrenme dönemindeki komplikasyon insidansının değerlendirilmesi

Özet
AMAÇ: Fakoemülsifikasyon yöntemi ile katarakt ekstraksiyonu yapılan olgularda gözlenen komplikasyonlar ve insidanslarının araştırılması. GEREÇ VE YÖNTEM: Mayıs1998-Haziran 2000 yılları arasında fakoemülsifikasyon cerrahisi uygulanan 128 hastanın 150 gözü çalışma kapsamına alındı. Tüm olgulara, 3.2 mm şeffaf korneal insizyon yapılarak fakoemülsifikasyon uygulandı. Olgular, intraoperatif ve postoperatif komplikasyonlar yönünden irdelendi. BULGULAR: İntraoperatif komplikasyonlar olarak, 21 gözde (%14) arka kapsül rüptürü ve vitreus kaybı, 19 gözde (%12.6) başarısız kapsüloreksis, 10 gözde (%6.6) düzensiz kesi, 9 gözde (%6) iris hasarı, 3 gözde (%2) intraoperatif kemozis, 2 gözde (%1.3) vitreus içerisine nukleus fragmanlarının düşmesi ve 1 gözde (% 0.6) ise Descemet dekolmanı ile karşılaşıldı. Postoperatif dönemde, 33 gözde (%22) geçici kornea ödemi, 14 gözde (% 9.3) geçici göz içi basıncı artışı, 9 gözde (%6) sektöriyel iris defekti, 3 gözde (%2) pupilla düzensizliği, 3 gözde (%2) kesi yerinden aköz sızıntısı, 2 gözde (%1.3) göz içi lensi malpozisyonu ve 1 gözde (%0.6) ise büllöz keratopati oluştuğu tespit edildi. SONUÇ: Uygun hasta seçimi ve cerrahi tecrübenin artması ile fakoemülsifikasyon cerrahisindeki komplikasyon oranları azalacaktır.
Abstract
PURPOSE: The purpose of this study is to evaluate the incidence of complication in cases who underwent cataract extraction by phacoemulsification method. MATERIAL AND METHODS: 150 eyes of 128 cases who had cataract extraction by phacemulsification from May 1998 to June 2000 were included in the study. We performed the phacoemulsification throught 3.2 mm clear corneal insicion. All cases were evaluated for the complications in operative and postoperative period. RESULTS: Posterior capsular tear and vitreous loss in 21 eyes (14 %), incomplete capsulorhexis in 19 eyes (12.6 %), irregular incision in 10 eyes (6.6 %), injury of iris in 9 eyes (6%), intraoperative chemosis in 3 eyes (2 %), dropping the nuclear fragments into the vitreous in 2 eyes (1.3 %) and detachment of Descemet in 1 eye (0.6 %) developed during operation. In postoperative period, temporary corneal edema in 33 eyes (22 %), temporary intraocular pressure rise in 14 eyes (9.3 %), sectorial defect of iris in 9 eyes (6 %), pupillary irregularity in 3 eyes (2 %), wound leakage in 3 eyes (2%), malposition of intraocular lens in 2 eyes (1.3 %) and bullous keratopathy in 1 eye (0.6 %) developed. CONCLUSION: It is concluded that the complication rates in phacoemulsification will reduce with experience and the selections of appropriate cases for surgery.
