Özet

Orbita hemanjiyoperisitomu, nadir görülen bir tümör olmasına rağmen, lokal rekürens, malign transformasyon, intrakranial invazyon ve uzak metastaz riski taşıması nedeniyle önemlidir. Olgumuzda, primer orbita hemanjiyoperisitomunun eksizyonunu takiben, 7 yıl sonra lokal rekürens ve malign transformasyon gelişmiştir. Tümör, kranial fronto-temporal girişimle tam ve intakt olarak çıkarılmış, cerrahiyi takiben rekürensin önlenmesi amacıyla toplam 6500 Rad radyoterapi uygulanmış, 40 aylık izlem süresinde rekürens gözlenmemiştir. Nüks orbita hemanjiyoperisitomunun ideal tedavi yaklaşımı, komplet intakt eksizyon ve postoperatif yeterli dozda radyoterapidir. Reküren tümör ve metastaz gelişme riski açısından, hastalar uzun süre izlenmelidirler.

Abstract

Although orbital hemangiopericytoma is a rare tumor, it is important because of the risk of local recurrence, malignant transformation, intracranial invasion and distant metastasis. In the present case study, our patient had a surgical excision of primary orbital hemangiopericytoma, 7 years later, local recurrence of tumor and malignant transformation occurred. An intact, complete excision was performed with cranial, fronto-temporal approach, then, applied a total dose of 6500 Rad radiotherapy to prevent recurrence and during 40 months follow up time, no recurrence was observed. The principal treatment of a recurrent orbital hemangiopericytoma consists of a complete-intact excision, and, postoperatively, an optimum dose of radiotherapy to prevent recurrences. Long term follow-up is mandatory as the tumor has high recurrence risk and the behaviour of the tumor is often unpredictable.
