Standart 30/2 ve fastpac 30/2 görme alanı testlerinin karşılaştırılması

Özet
AMAÇ: Normal ve glokomlu hastalara santral 30-2 standart eşik ve 30-2 fastpac eşik programını uygulayarak, fastpac programının klinik uygulamada kullanılabilirlik ve güvenilirliğini araştırmaktır. GEREÇ VE YÖNTEM: Tıbbi tedavi altındaki glokomlu 113 hastanın 171 gözü ile, glokomu olmayan sağlıklı 40 kişinin 57 gözünden oluşan toplam 153 kişinin 228 gözüne ait standart ve fastpac görme alanları retrospektif olarak incelendi. Olgular gruplara ayrılmadan ve Anderson'un tanımladığı kriterlere göre gruplara ayrılarak; test süresi, stimulus sayısı, ortalama deviasyon (MD), patern standart deviasyon(PSD), kısa süreli dalgalanma(SF), düzeltilmiş patern standart deviasyon(CPSD) kriterleri açısından değerlendirildi. İstatistiki analizler "paired t" ve "Wilcoxon Rank " testi ile yapıldı. BULGULAR: Olgular gruplara ayrılmadan incelendiğinde MD ortalaması standart testte -5.55 iken, fastpac testte -5.52 olarak bulundu ve fark istatistiki olarak anlamsızdı (p>0.05). PSD ortalaması standart testte 3.97 iken, fastpac testte 4.03 olarak bulundu, fark istatistiki olarak anlamsızdı (p>0.05). SF ortalaması standart testte 1.78 dB iken, fastpac testte 2.28 dB di; fark -0.5 dB di ve fark istatistiki olarak anlamlıydı (p<0.001). CPSD standart testte 3.16 iken, fastpac testte 2.74 bulunmuştur, fark istatistiki olarak anlamlıydı (p<0.001).Normal (kontrol ),başlangıç ,orta ve ileri dönem glokom olmak üzere gruplara ayrılan hastaların MD, PSD, SF ve CPSD parametreleri de karşılaştırılarak istatistiki analizler yapıldı. SONUÇ: Fastpac yöntemi standart teste göre daha az stimulusla daha kısa sürede sonuçlandı (%39.6). MD ve PSD açısından her iki yöntem arasında (normal grup hariç) fark bulunmadı. Fastpac testin SF değeri standart testten 0.5 dB daha fazladır, fakat fastpac test süresi standart testten daha kısaydı. Bağlantılı olarak fastpac testte CPSD değeri, standart testten daha düşüktü. Bu nedenle fastpac yöntemi glokom şüpheli veya glokomlu ve uzun süren testte hata yapan hastalarda kullanılabilir.
Abstract
PURPOSE: Standard and fastpac 30/2 visual field programs (VFP ) were compared in cases with glaucoma and normal individuals for determination of usability and safety of fastpac 30/2 visual field programs in clinical use. MATERIAL AND METHOD: Examination of standard and fastpac VFP of the 113 patients ( 171 eyes) who has been under medical treatment for glaucoma and 40 normal persons (57 eyes ), totally 153 persons (228 eyes), were retrospectively evaluated. All patients were evaluated for duration of the test, number of the stimulation, mean deviation (MD), patern standard deviation (PSD), short term fluctation (SF) and corrected patern standard deviation CPSD). Same parameters was determined in subgroups of the patients which were divided according to Anderson's criterias. Paired t and Wilcoxon Rank tests were used as statistically analyses. RESULTS: The average of the MD which determined by standard and fastpac VFP were -5.55 and -5.52, respectively. There was no statistically significant difference between these parameters. The average of the PSD of the all patients which determined by standard and fastpac VFP were 3.97 and 4.03, respectively. There was no statistically significant difference between these parameters. In all patients, the average of the SF for standard VFP was 1.78 dB, it was 2.28 dB for fastpac VFP. There was statistically significant difference between these parameters. The average of the CPSD in all patients for the standard VFP was 3.16, it was 2.74 for fastpac VFP. There was statistically significant difference between these parameters. In addition, these parameters were compared between normal individuals and patients with minimal, mild or severe glaucoma. CONCLUSION: Number of the stimulation and duration of the test were less in fastpac VFP than standard VFP. For all patients except normal group there was no difference between these tests for values of MD and PSD. Average of the SF for fastpac test 0.5 dB more than standart test, but test duration of the fastpac was less than standart test. In addition, value of the CPSD was less in fastpac test than standard test. For these reasons, fastpac VFP are used in patients with glaucoma, suspect of the glaucoma and for patients who make mistake at long test time.
