Özet

Modern katarakt cerrahisinde, birçok avantajından dolap arka ka¬mara lenslerinin (AKL) kapsül içerisine yerleştirilmesi tercih edil¬mektedir. Ancak, ameliyat sırasında bu amaca ulaşıldığı düşünülse de, postoperatif dönemde kapsülün yırtılması sonucu bazen lens kapsül dışına çıkabilmektedir. Kliniğimizde, hepsi zarf tekniği ile açılarak planlanmış ekstrakapsüler katarakt ekstraksiyonu yapılan ve AKL yerleştirilen olguların postoperatif dönemdeki lens konum¬larım ve diğer klinik bulgularını değerlendirmek amacıyla 45 has¬taya ait 56 göz incelenmiştir. AKL'nin pozisyonu, biyomîkroskopi ve Goldmann'ın üç aynalı kontakt lensi kullanılarak değer¬lendirilmiştir. AKL'nin kapsül içi yerleşiminin, olguların %59'unda sağlanabildiği gözlenmiştir. Olguların %12.5'inde sulkusta ve %23.2'sinde alt kenan kapsül içinde olmasına karşın üst kenarının sulkusta olduğu saptanmış ve bu durum asimetrik yerleşim olarak adlandırılmıştır. Hastalar ayrıca postoperatif astigmatizma yönün¬den değerlendirilmiş, lensin konumu ile astigmatizma arasında an¬lamlı bir ilişki saptanamamıştır (P>0.05). AKL'nin asimetrik yerleşimi desantralizasyon riskini arttırmaktadır ve kapsülün yırtılma olasıhğı ön kapsülektomi ve kullanılan lens ti¬pi ile yakından ilişkilidir.

Abstract

Postoperative Evaluation of the Posterior Chamber Lenses Implanted in the Capsular Bag In modern cataract surgery, implantation of the pos¬terior chamber lenses into the capsular bag is prefer¬red because of its many advantages, although it is as¬sumed that this purpose is achieved in intraoperative period, sometimes in postoperative period intraocular lenses are observed located outside the capsular bag as a result of capsular rupture. To evaluate postope¬rative intraocular lens position and other clinical fea¬tures, we examined 56 eyes of 45 patient who under¬went planned extracapsular cataract extraction with envelop capsulectomy and posterior chamber lens implantation. Position of the IOL was determined using biomicroscopy and Goldmann three-mirror contact lens. We observed that capsular. bag fixation of the IOL occured in %59 of cases. In %12.5 of cases IOL was in the ciliary şulcus and in %23.2 öf cases inferior edge was in the capsular bag while the superior edge was in the ciliary sulcus, which is cal¬led asymmetrical situation. We also examined the postoperative astigmatism of the patient and we didra't find any significant correlation between the lens position and the astigmatism (P>0.05). Asymmetrical situation of the IOL increases the risk of decentration, and the incidence of capsular rupture is related with the type of anterior capsulectomy and the IOL which is used.
