Tümörleri ve flep dokularında p53 gen mutasyonu ve erbB2 protoonkogen amplifikasyonu

Özet
AMAÇ: Göz kapağı tümörü nedeniyle cerrahi uygulanan olgularda; tümöral doku, eksizyon sonrası rekonstrüksiyonda kullanılan komşu flep bölgesi ve ultraviyole (UV) ile temas etmeyen ön kol derisinde tümör süpresör geni p53 ve erbB2 protoonkogeni araştırıldı. GEREÇ VE YÖNTEM: 3 bazal hücreli karsinom, 1 hidroadenom, 1 sebase gland karsinom ve 1 senil keratoz histopatolojik tanısı alan 6 olguda tümör dokusu, flep bölgesi ve ön koldan alınan doku örnekleri p53 (tümör süpresör gen) ve erbB2 protoonkogenlerine spesifik problar kullanılarak FISH (Floresan In Situ Hibridizasyon) tekniği ile değerlendirildi. BULGULAR: Tümör supresör gen olan p53 geni için yapılan FISH analizlerinde olguların 5'inde tümör dokusunda %23-38 oranında tek ya da çift allel kaybı gözlenmiş olup kesim noktası (cut-off) değerinden yüksek olmaları nedeniyle anlamlı kabul edildi. Buna karşılık bu olguların normal dokularında p53 delesyonu yoktu. Senil keratozlu olgunun kitle dokusu ve normal dokularında p53 delesyonu görülmedi. erbB2 amplifikasyonu incelendiğinde; hidroadenomlu, senil keratozlu ve bazal hücreli karsinomlu bir olguda erbB2 amplifikasyonu azdı, tümör dokusuyla sağlam dokular arasında fark yoktu. Diğer 3 olguda ise erbB2 amplifikasyonu %25, %30 ve %14 oranında görüldü. Bu olguların diğer örneklerinde ise normal sinyal fenotipi ile karşılaşıldı. SONUÇ: Tümör dokularıyla flep çevrilen bölge ve ön koldan alınan doku örnekleri arasında tümör oluşumunda etkili olan genler bakımından farklılıkların olduğu görüldü. Ancak flep çevrilme bölgesi ile ön koldan alınan dokular arasında fark bulunmadı. Bu da rekonstrüksiyonda, tümör dokusu ile aynı dış etkenlere maruz kalan komşu bölgeden çevrilen fleplerin güvenle kullanılabileceğini göstermektedir.
Abstract
PURPOSE: To search for tumor suppressor gene p53 and erbB2 protooncogene in tumoral tissues, adjacent healthy skin, and forearm skin samples used for reconstruction after excision in lid tumors. MATERIAL AND METHODS: Tumor tissue samples from three cases with basal cell carcinoma, one with hydroadenoma, one with sebaceous gland carcinoma and one with senile keratosis and skin samples from adjacent healthy flap, and forearm were analysed for p53 (tumor supressor gene) and erbB2 (protooncogene) mutations by the standard fluorescent in situ hybridisation (FISH). RESULTS: FISH analyses for the tumor supressor gene p53 showed single or double allele loses in the range of 21-38% in five cases, whereas the healthy tissues showed no p53 deletions. The frequency of mutant nuclei was higher than the cut-off value and it was considered as significant. Tumor and healthy tissues in case with senile keratosis showed no p53 deletions. However in one of the cases with basal cell carcinoma and in the cases with hydroadenoma and senile keratosis erbB2 amplification was less. There was no difference between tumoral and healthy tissues with respect to the protooncogene. In the remaining three cases, the erbB2 amplification were 25%, 30% and 14%. The healthy tissues of these cases revealed normal signal phenotypes. CONCLUSION: Results show difference with respect to genes participating in tumorigenesis among tumoral tissues samples and skin samples from adjacent flaps and forearm. No difference was found also in samples from flaps and forearm. This suggest that flaps exposed to the same external influences as the tumoral tissue are equally safe for using in reconstruction with free grafts taken from the forearm away from the lesion.
