Özet
Amaç: Erişkin yaş grubundaki travmatik glokom olgularımızın, hasta yaş, cinsiyet, travma etiyolojisi, tedavi prensipleri ve hastalık prognozu açısından dökümünü yapmak. Gereç ve Yöntem: Aralık 1999-Mart 2003 dönemi süresince, SSK Ankara Göz Eğitim Hastanesi II. Göz Kliniği Glokom Birimi'nde takip edilen 42 olgunun, travmatik glokomlu 42 gözü çalışma kapsamına alındı. Otuz erkek (%71.4), oniki kadın (%28.6) olgunun yaş ortalaması 18.1±11.1(5-38 yaş) bulundu. Bulgular: Etiyolojide, 28 gözde (%66.7) künt travma, 14 gözde ise (%33.3) delici travmaya rastlandı. Travma gelişiminden glokom teşhis edilene dek geçen süre, künt travmalarda 3.8±3.4 ay (1-14 ay), delici travmalarda 5.9±4.6 ay (1-16 ay), künt ve delici travma genelinde ise 4.5±3.9 ay (1-16 ay) olarak bulundu. Glokom teşhis esnasında göziçi basıncı, künt travmalarda 25.5±3.1 mmHg (22-36 mmHg), delici travmalarda 26.3±3.3 mmHg (23-33 mmHg), künt ve delici travma genelinde ise 25.7±3.2 mmHg (23-36 mmHg) olarak bulundu. Künt travma geçiren 28 gözün 12'sinde, tıbbi antiglokomatöz tedaviye yanıt alınırken; 16 göze bir kez, 5 göze ise ikişer kez mitomisin-c ile trabekülektomi yapıldı. Delici travma öyküsü olan 14 gözün ise; 8'inde göziçi basıncı tıbbi tedavi ile kontrol altına alınırken, 6 gözde ise bir veya ikişer kez mitomisin-c ile trabekülektomiye gidildi. Sonuç: Travma sonrasında, çeşitli mekanizmalarla gelişen sekonder glokom riski akılda tutulmalıdır
Abstract
Purpose: To evaluate the age and sex of the patients, ethiology of trauma and the prognosis of therapy in posttraumatic glaucoma cases. Material and Methods: A total of 42 eyes of 42 patients with posttraumatic glaucoma which had been seen between January 1999 and March 2003 in Ankara Social Security Eye Hospital Galucoma Department, were included in this study. The mean age was 18.1±11.1(5-38 years) in 30 male (71.4%) and 12 female (28.6%) patients. Results: In 28 eyes (66.7%) contusion injuries, in 14 eyes (33.3%) penetrating injuries were detected. The period between the occurrence of trauma and the diagnosis of glaucoma was 3.8±3.4 months (1-14 months) in contusion injuries, 5.9±4.6 months (1-16 months) in penetrating injuries and 4.5±3.9 months (1-16 months) in both types of injuries. The intraocular pressure at the time of the diagnosis of glaucoma was 25.5±3.1 mmHg (22-36 mmHg) in contusion injuries , 26.3±3.3 mmHg (23-33 mmHg) in penetrating injuries and 25.7±3.2 mmHg (23-36 mmHg) in both types of injuries. The intraocular pressure was controled in 12 of 28 eyes with contusion injuries with antiglaucomatous topical agents but, in 16 eyes primary, in 5 eyes secondary trabeculectomies with mitomicin-c were done. In 8 of 14 eyes with penetrating injuries, topical agents were enough to control the intraocular pressure but, in 6 eyes primary and secondary trabeculectomies with mitomicin-c were done. Conclusion: The risk of secondary glaucoma should not be forgotten after trauma.
