Özet

Katarakt ekstraksiyonu ve arka kamara göz içi lensi (GİL) implantasyonu yapılan hastalarda, GİL'nin pozisyonunun ultrason biyomikroskopi (UBM) ile değerlendirilmesi. Gereç ve Yöntem: Kliniğimizde Haziran 2001-Kasım 2002 tarihleri arasında katarakt cerrahisi ugulanan 42 hastanın 45 gözü UBM ile değerlendirildi. Hastaların yaşlan 45 ile 79 arasında değişmekte olup (ortalama 64 yaş), olguların 20 'si kadın, 22 'si ise erkekti. Lens ekstraksiyonu 32 gözde limbal kesi, zarf kapsülotomi ve ekstrakapsüler katarakt ekstraksiyonu (EKKE) yöntemi kullanı larak, 13 gözde korneal kesi, kapsüloreksis ile ön kapsülotomi ve fakoemulsifikasyon yöntemi ile yapılmış ve arka kamara GİL'inin kapsül içine implante edilmesi amaçlanmıştı. Tüm görüntülemeler 50 MHz transducer kullanılarak 80 dB/mm gain, 5 dB/mm time-gain ayarında yapıldı. Santral kornea görüntülendikten sonra periferik kornea 360 derece, limbusa dik olarak görüntülendi. Bulgular: Ekstrakapsüler katarakt ekstraksiyonu uygulanan 32 gözün 13 'ünde her iki lens haptiği kapsül içinde (%40.6), 14 'ünde alt haptık kapsül içinde, üst haptık sulkusda (%43.8), 3 'ünde her iki lens 'haptiği sulkusda (%9.4), l 'inde üst haptik kapsül içinde, alt haptik siliyer sulkusda (%3.1) ve l 'inde üst haptik siliyer cisime disloke olmuş şekilde, alt haptik kapsül içinde izlendi (%3.1). Fakoemulsifikasyon cerrahisi uygulanan 13 gözün 13'ünde de (%100) her iki lens haptiği kapsül içinde tespit edildi Sonuç: Arka kamara GİL 'nin her iki haptiğinin kapsül içine implantasyon oranı kapsüloreksis ile ön kapsülotomi yapılan gözlerde, zarf kapsülotomi tekniği ile ön kapsülotomi uygulanan gözlere göre daha yüksektir.

Abstract

Purpose: To evaluate the position of the posterior chamber intraocular lenses (IOL) by ultrasound Biomicroscopy (UBM) in patients -who had cataract extraction and posterior chamber IOL implantation. Material and Method: Forty-five eyes of 42 patients who had cataract surgery between June 12001-November 2002 were evaluated by UBM. The patients ranged in age from 45 to 79 years (mean: [64 years). Twenty patients were female and 22 were male. Lens extraction was performed by extracapsular cataract extraction (ECCE) using envelope capsulotomy and limbal incision in 32 eyes andfind by phacoemulsification with capsulorhexis and corneal incision in 13 eyes. In all eyes the posterior chamber IOL was aimed to be implanted in the capsular bag. The imaging was performed using a 50 MHz transducer at 80 dB/mm gain and 5 dB/mm time-gain. After the central cornea was imaged, peripheral cornea was imaged 360 degrees perpendicular to limbus. Results: In 32 eyes that had ECCE surgery, both of the lens haptics were located in the capsular bag in 13 eyes (%40.6), inferior haptic was located in the capsular bag and the superior haptic was located in the sulcus in 14 eyes (%43.8), both haptics were located in the sulcus in 3 eyes (%9.4), inferior haptic was located in the sulcus and the superior haptic was located in the capsular bag in 1 eye (%3.1), and inferior .haptic was located in the capsular bag and the superior haptic was located in the ciliary body in 1 eye (%3.1). In 13 of the 13 eyes (%100) who had phacoemulsification both of the lens haptics were localized in the capsular bag (p<0.001). Conclusion: The implantation of the both haptics of the posterior chamber IOL in the capsular bag is more frequently achived using capsulorhexis than envelope capsulotomy.
