Granülomatöz üveitli bir olguda intraoküler lens eksplantasyonu

Özet
Üveit atakları geçirmiş bir hastada, intraoküler lens (IOL) implantasyonusonrası gelişen tedaviye dirençli granülomatöz üveit tedavisini tartışmak, katarakt cerrahisi sonrası İOL eksplantasyon endikasyonlarını tartışmak.Uzun yıllardır üveit tanısıyla takip edilen, iki yıl önce sol gözünden katarakt cerrahisi geçiren ve ameliyat sonrası tedaviye dirençli üveit sebebiyle tedavi edilen yetmişiki yaşındaki bayan hastanın sol gözde görme keskinliği 20/200 düzeyinde tespit edildi. Ön segment muayenesinde endotelde yaygın granülomatöz keratik presipitatlar, ön kamarada 3+ hücre, açı bölgesinde hücre birikintileri izlendi. Medikal tedaviye cevapsız olması nedeniyle IOL eksplantasyonu yapılan hastanın postoperatif 1. haftada görmesinin afak tashihle 20/30 düzeyine çıktığı, keratik presipitatların kaybolduğu görüldü. Açı bölgesindeki depozitlerden yapılan histopatolojik incelemede epiteloid histiositik topluluklar tespit edildi.Üveit hikayesi bulunan hastalarda, katarakt cerrahisi sonrası tedaviye dirençli üveit atağı görülebilir. Bu hastalarda erken dönemde IOL eksplantasyonu sekonder komplikasyonların önlenmesi açısından önemlidir
Abstract
To discuss the treatment of granulomatous uveitis after intraocular lens (IOL) implantation in a patient who had a complaint of recurrent uveitis, and to assess the indications of intraocular lens explantation after cataract surgery. A seventy-two-year-old female who had been followed for uveitis for many years and who had had a cataract surgery for her left eye, suffered from resistant uveitis for two years after IOL implantation and had a left visual acuity of 20/200. Slit-lamp examination revealed diffuse endothelial granulomatous keratic precipitates, 3+ aqueous cells and cellular precipitates in angle region. She underwent IOL explantation because of ineffectiveness of the medical therapy. She reached a best corrected visual acuity of 20/30 in the first week postoperatively and the keratic precipitates disappeared. Histopathology of the angular precipitates revealed epitheloid histiocytic collection. Resistant uveitis can be seen after cataract surgery in patients with a history of uveitis. Early IOL explantation may prevent secondary complications in these cases.
