Diabetes Mellitus Hastalarında Fakoemülsifikasyon Cerrahisi Sonrasında Gelişen Kapsül Kontraksiyonunu Etkileyen Faktörler

Özet
Amaç: Fakoemülsifikasyon cerrahisi uygulanan diabetes mellitus (DM) hastalarında gelişen kapsül kontraksiyonuna etki eden sistemik, oküler ve cerrahi faktörlerin incelenmesi.Gereç ve Yöntem: Fakoemülsifikasyon cerrahisi uygulanan 27 DM hastasının 27 gözü(DM grubu) ile yaş ve cinsiyet yönünden DM grubuna benzer özellikteki 26 sağlıklı katarakthastasının 26 gözüne (kontrol grubu) ait kapsüloreksis alanlarının postoperatif 1.gün, 3.ay ve6.aylardaki değişim oranları karşılaştırıldı. Ayrıca, retinopati varlığı, diyabetin metabolikkontrolü, kapsüloreksisin başlangıç çapı, nükleus sertliği ve fakoemülsifikasyon zamanı gibisistemik, oküler ve cerrahi faktörlerin bu süreç üzerine etkileri incelendi. Bulgular: Kapsüloreksis alanlarının başlangıç değerlerinin ortancası DM grubunda16,5mm2(12,6-29,2), kontrol grubunda ise 16,3mm2(11,3-30,7) idi (p>0,05). Her iki gruptada kapsüloreksis alanının 6. aya kadar anlamlı olarak azalmaya devam ettiği saptandı(p 0,05).Sonuç: Diyabetli hastalarda fakoemülsifikasyon cerrahisi sonrasında kapsül kontraksiyonu sağlıklı katarakt olgularına göre daha şiddetli olarak gelişmektedir. Bu süreç postoperatifilk 3 ayda belirgindir. Oküler, sistemik veya cerrahiye ait faktörlerin bu süreç üzerinde etkiliolmadığını saptamış olmamıza rağmen, postoperatif inflamasyon şiddetinin dikkate alınacağı ileri çalışmalar olası ilişkilerin netleştirilmesinde yardımcı olabilir
Abstract
Aim:To investigate systemic, ocular, and surgical factors acting upon capsular contrac-tion which occurs after phacoemulsification in patients with diabetes mellitus (DM). Material and Method: The variation of the capsulorhexis area with regard to the meas-urements at the 1stday, 3rdmonth, and 6thmonth after phacoemulsification surgery are com-pared in two groups of patients, one of which included 27 eyes of 27 cases with DM (DMgroup), and the second group which consisted of 26 eyes of 26 otherwise healthy cataract pa-tients with similar age and sex distribution (control group). Systemic, ocular, and surgical fac-tors such as, metabolic control of diabetes, existence of diabetic retinopathy, as well as, ini-tial diameter of capsulorhexis, hardness of lens nucleus, and duration of phacoemulsification,which might have an impact of this course were evaluated. Results: The medians of the capsulorhexis areas in DM, and control groups were 16.5 mm2 (12.6-29.2), and 16.3 mm2(11.3-30.7), respectively (p>0.05). Capsulorhexis areas continuedto decrease till the 6thmonth in both groups (p<0.001). The contraction rates of the capsu-lorhexis areas were significantly higher in the DM group compared to the control group, fromthe 1stday to the 3rdmonth, as well as to the 6thmonth (p<0.001). However, no significant dif-ference was found between the two groups with regard to the shrinkage in rhexis area betweenmonth 3 and 6 (p=0.223). Other systemic, ocular, or surgical factors were not found to sig-nificantly influence the alterations of capsulorhexis areas at different time points (p>0.05 foreach parameter). Conclusion:Capsular contraction develops more evidently in patients with DM comparedto otherwise healthy individuals with cataract. The process is more pronounced in the first 3months. Although, systemic, ocular, or surgical factors were not found to effect the changecourse, further studies taking the extent of postoperative inflammation into considerationmight help to reveal any possible relationships
