Bakteriyel keratitlerde kombine tedavi ve monoterapinin etkinliğinin karşılaştırılması

Özet

Amaç: Kombine tedavi veya monoterapi ile tedavi ettiğimiz bakteriyel keratitli olgularımı- zın sonuçlarını karşılaştırmayı amaçladık. Gereç ve Yöntem: Çalışma kriterlerine uyan 83 bakteriyel keratitli hastanın (83 göz) dos- yası retrospektif olarak incelendi ve olgular iki gruba ayrıldı. Grup 1 (n=41) ve Grup 2 (n=42). Grup 1’e %5 sefazolin ve %1,3 gentamisin kombinasyonu ile tedavi edilen olgular alındı. Grup 2 hastaları gatifloksasin (n=26) veya moksifloksasin (n=16) ile monoterapi uy- gulananlardı. Bulgular: Gruplar cinsiyet (p=0,938), yaş ortalaması (p=0,492), ortalama takip süresi (p=0,509), başlangıç görme keskinliği (0,05), ülser genişliği (p=0,690), ülser yerleşimi (p=0,993), ülser derinliği (p=0,822), ön kamara reaksiyonu (p=0,741) ve hipopiyon (p=0,746) bakımından benzerdi. Ülserin üzerindeki epitelin ortalama iyileşme süresi Grup 1’de 11,9±4,78 gün ve Grup 2’de 13,26±4,88 gündü (p=0,211). Tedavi sonrası ortalama gör- me keskinliği Grup 1’de 0,351±0,346 ve Grup 2’de 0,486±0,418 olarak bulundu (p=0,235). Grup 1’de 24 (%58,5) gözde, Grup 2’de 21 (%50,0) gözde kültürler pozitifti (p=0,435). Kültürde en sık üretilen bakteri Staphylococcus aureus idi (n=12, %14,4). Grup 1’de Grup 2’ye oranla daha sık yan etki ortaya çıktı (p=0,037). Sonuç: Bakteriyel keratitler dördüncü kuşak florokinolonlarla tedavi edildiğinde sefazolin ve gentamisin kombinasyonu ile tedavi edilenlerle benzer sonuçlar elde edilmektedir. Dördüncü kuşak florokinolonların avantajı, kombine tedaviye göre daha seyrek yan etkiye yol açmalarıdır.

Abstract

Aim: The purpose of this study was to evaluate the effects of combined treatment and monotherapy in bacterial keratitis patients. Material and Method: The records of 83 patients with bacterial keratitis were evaluated retrospectively and patients were divided into two groups: Group 1 (n=41) and Group 2. Group 1 was comprised of patients who were treated with a 5% cefazolin and 1.3% gentam- icin combination. The Group 2 patients were treated with gatifloxacin (n=26) or moxifloxacin (n=16) monotherapy. Results: The groups were similar regarding the gender (p=0.938), mean age (p=0.492), mean follow-up time (p=0.509), initial mean visual acuity (0.05), ulcer diameter (p=0.690), ulcer localization (p=0.993), ulcer depth (p=0.822), anterior chamber reaction (p=0.741) and hypopyon (p=0.746). The mean epithelization time was 11.9±4.78 in Group 1 and 13.26±4.88 in Group 2 (p=0.211). At the last visit, the mean visual acuity was 0.351±0.346 in Group 1 and 0.486±0.418 in Group 2 (p=0.235). The side effects were observed more frequently in Group 1 than in Group 2 (p=0.037). Conclusion: The same outcomes were achieved when bacterial keratitis patients were treated with a cefazolin/gentamicin combination or with gatifloxacin or moxifloxacin monotherapy. The advantage of fourth generation fluoroquinolones was the lesser extent of drug side effects.
