İnferior oblik kasın anterior transpozisyonunun vertikal kapak aralığına etkisi

Özet
Amaç: (İO) kasın anterior transpozisyonu cerrahisinin vertikal kapak aralığı üzerine etkisini araştırmak. Gereç ve Yöntem: Bu prospektif çalışma aşikar disosiye vertikal deviyasyonun tedavisi için; 19unun tek taraf İO kasın anterior transpozisyonuna maruz kaldığı (çalışma grubu), 16sının tek taraf İO kas geriletmesine maruz kaldığı (kontrol grubu) 35 hastadan oluşmak- taydı. İO kas cerrahisi her bir gözde tek cerrahiydi. Her iki grupta vertikal kapak aralıklarındaki değişiklikler preoperatif ve postoperatif fotoğraflar karşılaştırılarak değerlendirildi. Bulgular: Cerrahi sonrası ortalama vertikal kapak aralığının daralması kontrol grubunda 0,2±0,1 mm, çalışma grubunda 1,5±1,1 mm idi. Çalışma ve kontrol grupları arasındaki farklar istatistiksel olarak önemliydi (p<0,001). Sonuç: İnferior oblik kasın anterior transpozisyon cerrahisi, özellikle tek taraflı tedavi edilen vakalarda kozmetik olarak fark edilebilen, vertikal kapak aralığının daralmasına sebep olabilir.
Abstract
Aim: To investigate the effect of anterior transposition procedure of the inferior oblique (IO) muscle on the vertical palpebral fissure. Material and Method: This prospective study consists of 35 patients, of whom 19 under- went unilateral IO anterior transposition (study group) and 16 underwent unilateral IO muscle recession (control group) for the treatment of manifest dissociated vertical deviation (DVD). The IO muscle surgery was the sole surgical procedure in each eye. The changes in the vertical palpebral fissure were evaluated comparing preoperative and postoperative photo- graphs in two groups. Results: The mean narrowing of the vertical palpebral fissure after surgery was 0.2±0.1 mm in the control group, 1.5±1.1 mm in the study group. The differences were statistically significant between the control and the study groups (p<0.001; t test). Conclusion: The anterior transposition operation of the IO muscle may cause narrowing of the vertical palpebral fissure, which can be cosmetically noticeable, particularly in patients treated unilaterally.
