Özet

Doğal gözyaşının eşsiz içeriği, oküler yüzey homeostazisi için vazgeçilmezdir. Gözyaşı eksikliği, kornea ve konjonktiva epiteliyle başlayan doku bütünlüğünü, enfeksiyonlara yönelik doğal direnci, optik pürüzsüzlükle sağlanan net görmeyi bozar.Ayrıca, serbest sinir ucu yoğunluğu nedeniyle her göz kırpmada yaşam kalitesini de olumsuz yönde etkiler. Yapay alternatifibulunmayan bu zengin içerikli sıvı, ne mutlu ki, bir başka zengin içerikli doğal vücut sıvısı olan kan kökenli sıvılarla yerinekonabilecek durumdadır. Tıp literatüründe yer almasa da, yazılı literatüre geçmiş olan ve 6 asır önce kaleme alınmış DedeKorkut Kitabı'ndaki Bamsı Beyrek Öyküsü'nde ilk kez tarif edilen kan serumunun göz yüzeyine uygulanması yoluyla göz sağlığının yeniden kazanılması, modern tıbbın özellikle 1999'dan sonra çokça üzerinde durduğu bir başlık olmuştur. Bu derlemede, kan serumu başta olmak üzere, plazma, kordon kanı serumu, trombosit konsantreleri ve albümin kan ürünü gözyaşıtakviyesi seçenekleri olarak ele alınmış, onları zengin kılan epitelyotrofik faktörler kısaca tanıtılmıştır

Abstract

The unique composition of natural tears is essential for ocular surface homeostasis. Tear deficiency deteriorates the tissueintegrity of the cornea and conjunctiva, natural immunity towards infections, and the clear vision provided by smooth opticalsurfaces. Furthermore, due to the high density of free nerve endings in the corneal surface, tear deficiency negatively affectslife quality with each blink. This rich fluid has not yet to be matched by artificial alternatives; fortunately, other natural, richbodily fluids can be derived from the blood and used in its stead. Although not represented in the medical literature, the recovery of eye health through the application of blood serum to the ocular surface was first described six centuries ago inwritten literature, in the Tale of Bamsi Beyrek, a story contained in the Book of Dede Korkut. The topic of blood-derived fluids as ophthalmic therapy has gained interest to modern medicine since 1999. This review discusses the current blood-derived tear substitute options, including blood serum, plasma, umbilical cord serum, platelet concentrates and albumin, andbriefly describes the epitheliotropic factors that make these options valuable as tear substitutes
