Özet

Amaç: Açık açılı glokomun cerrahi tedavisinde gonyoskopi yardımlı sütür trabekülotomi tekniğinin göz içi basıncını azaltmadaki ön sonuçları değerlendirmek. Gereç ve Yöntem: Hastaların görme keskinlikleri belirlendi. Biyomikroskopik ön segment ve arka segment incelemesi yapıldı. Gonyoskopi ile açı muayenesi yapılıp, göz tansiyonu aplanasyon tonometrisi ile ölçüldü. Maksimal medikal tedavi ile kontrol altına alınamayan 19 açık açılı glokom hastasının yirmi gözüne aynı cerrah tarafından gonyoskopi yardımlı sütür trabekülotomi uygulandı. Swan-Jacop gonyolens yardımı ile MVR mikrocerrahi bıçak ile nazal açıda trabekülüm üzerinden gonyotomi oluşturuldu. Mikrocerrahi forsepsle ucu köreltilmiş sütür gonyotomi alanı kullanılarak Schlemm kanalına sokuldu. Sütür kanal boyunca 360O ilerletilip, trabekülotomi tamamlandı. 9 olgu fakoemülsifikasyon ile kombine edildi. Göz içi basıncının ilaçsız olarak 18 mmHg’den az olması durumunda, cerrahi başarı “tam” olarak tanımlandı. İlaçlarla aynı göz içi basıncı düşüşü elde edildiğinde ise cerrahi başarı “kısmi” olarak tanımlandı. Bulgular: On beş erkek ve dört kadın hastanın yaş ortalaması 60,7 (29-79) idi. Olguların 13’ü primer açık açılı, 6’sı psödoeksfolyatif ve 1 göz pigmenter glokom idi. Hastaların ameliyat sonrası 1. gün,1. hafta, 1. ay, 2. ay, 3. ay, 6. ay ve 12. ayda muayeneleri yapıldı. Ameliyat öncesi ortalama göz içi basıncı 21,7± 7,1mmHg, glokom ilaç sayısı ise ortalama 4,0 idi. Ameliyat sonrası ortalama göz içi basıncı ve ilaç sayısı sırasıyla 1. ayda 14,2±6.6mmHg ve 0.0 bulundu. 3. ayda 12,3±4,4mmHg ve 2,0, 6. ayda 11,1±3,6mmHg ve 2.5, 12. ayda ise 11,3±2,8mmHg ve 2.0 bulundu. Göz içi basıncı ve ilaç sayısının azalması istatistiksel olarak anlamlı idi (her biri için p<0,05). Gonyoskopi yardımlı sütür trabekülotomi ve kombine (Fako+gonyoskopi yardımlı sütür trabekülotomi) cerrahi girişimler arasında göz içi basıncı açısından istatistiksel olarak anlamlı fark bulunmadı (p=0,46). Takip süresince 2 olguya ek filtrasyon cerrahisi gerekti. Sonuç: Gonyoskopi yardımlı sütür trabekülotomi, açık açılı glokomda göz içi basıncı düşüşü için etkili bir tedavi olarak ümit verici bir tekniktir.

Abstract

Objective: To evaluate the preliminary results of gonioscopy-assisted suture trabeculotomy technique in reducing intraocular pressure in surgical treatment of open-angle glaucoma. Material and Method: Visual acuity of the patients was determined. Biomicroscopic anterior segment and posterior segment were examined. Angle examination was performed by gonioscopy and eye tension was measured by applanation tonometry. Twenty eyes of 19 open-angle glaucoma patients whose intraocular pressure could not be controlled with maximum medical therapy underwent gonioscopy-assisted suture trabeculotomy by the same surgeon. A goniotomy was performed over the trabeculum at nasal angle with MVR microsurgical knife using SwanJacop gonyolens. The tip blunted suture was inserted into Schlemm canal using microsurgical forceps and goniotomy area. The suture was advanced 360 degrees along the canal and trabeculotomy was completed. Nine patients’ treatment was combined with phacoemulsification. Surgical success was defined as “full success” if intraocular pressure was less than 18 mmHg without medication. Surgical success was defined as “partial success” if intraocular pressure was less than 18 mmHg with medication. Results: The mean age of the fifteen male and four female patients was 60.7 (29-79) years. Thirteen of the cases were primary open angle, 6 eyes were pseudoexfoliative, and 1 eye was pigmentary glaucoma. Patients were examined first day after the operation. Later patients were examinated first week, 1st month, 2nd month, 3rd month, 6th month and 12th month. The mean preoperative intraocular pressure was 21.7±7.1 mmHg and the number of glaucoma medication was 4.0. The mean postoperative intraocular pressure and the number of drugs were 14.2 ± 6.6mmHg and 0.0 at the first month, respectively. The mean postoperative intraocular pressure and the number of drugs were found 12,3±4.4mmHg and 2.0 at the 3rd month; 11,1±3,6mmHg and 2.5 at the 6th month; 11.3±2.8mmHg and 2.0 at the 12th month, respectively. The reductions of intraocular pressure and number of medication were statistically significant (p<0.05, for each). No statistically significant difference in terms of intraocular pressure was found between gonioscopy-assisted suture trabeculotomy and combined (Phaco+ gonioscopy-assisted suture trabeculotomy) surgical procedures (p=0.46). Additional filtration surgery was required in 2 cases during follow-up. Conclusion: Gonioscopy assisted suture trabeculotomy is a promising technique as an effective treatment for intraocular pressure reduction in open-angle glaucoma.
