Kronik hepatit B ve hepatit C saptanan hastalarda ekstrahepatik hastalıkların sıklığı: 435 hastanın retrospektif olarak incelenmesi

Özet
Hepatit B ve C viruslarının neden olduğu kronik enfeksiyonlar, tüm dünyada önemli bir halk sağlığı sorunudur. Ülkemizde alınan tüm kontrol önlemlerine rağmen, kronik hepatit B ve C enfeksiyonlarının sıklığı sırasıyla %5-7 ve %3 arasında değişmektedir. Yapılan çalışmalar, hepatit B ve C enfeksiyonlarına, Diabetes Mellitus (DM), hipo- ya da hiper-tiroidi, Behçet Hastalığı (BH) ve otoimmün hastalıkların eşlik ettiğini göstermektedir. Bu çalışmada, kronik B hepatiti (KBH) olan 400 ve kronik C hepatiti (KCH) olan 35 hasta, ekstrahepatik hastalıkların sıklığınının araştırılması amacıyla retrospektif olarak incelenmiştir. KBH ve KCH hastalarında saptanan oranlar sırasıyla; DM için %1.5 ve %8.5, hipotiroidi için %1.5 ve %5.7, hipertiroidi için %0.75 ve %2.8, noduler guatr için %2 ve %20, BH için %0.25 ve %5.7, Sistemik Lupus Eritematozus için %1 ve %0, Romatoid Artrit (RA) için %1 ve %5.7 ve demir eksikliği anemisi için %22 ve. %11.4 olarak belirlenmiştir. Her iki hasta grubunda da en sık saptanan hastalıkların DM, tiroid bozuklukları ve RA olduğu belirlenmiş, KCH olan hastalarda DM, BH ve noduler guatr'ın, KBH olanlardan daha yüksek oranda görüldüğü dikkati çekmiştir (p<0.05). Sonuç olarak, kronik hepatit B ve C virüs enfeksiyonlarında ekstrahepatik bulgu ve belirtilerin dikkatle takip edilmesi, erken tanı ve uygun tedavi açısından yararlı olacaktır.
Abstract
Chronic infections that caused by hepatitis B and C viruses, are important ali of the world as a public health problem. Despite intensive controlmeasurements, the incidences of chronic hepatitis B and C virüs infections in our country are estimated as 5-7% and 3%, respectively. There are accumulating data of especially autoimmune mediated extrahepatic manifestations of hepatitis B and C virüs infections, such as Diabetes Mellitus (DM), Behçet's Disease (BD), hypo-or hyper-thyroiditis and autoimmune diseases. The aim of this study was the retrospective evaluation of 400 chronic B hepatitis (CBH) and 35 C hepatitis (CCH) patients by means of the presence of extrahepatic manifestations. As a result, the positivity rates of extrahepatic manifestations detected in CBH and CCH patients were as follovvs respectively; 1.5% and 8.5% for DM, 1.5% and 5.7% for hypothyroiditis, 0.75% and 2.8% for hyperthyroiditis, 2% and 20% for nodular goitre, 0.25% and 5.7% for BD, 1 % and 0% for systemic lupus, 1 % and 5.7% for Rheumatoid Arthritis (RA), and 22% and 11.4% for iron deficiency anemia. DM, thyroid diseases and RA were the most common extrahepatic manifestations detected in both of the patient groups, while the rates of DM, BD and nodular goitre were found higher in CCH patients than CBH patients (p<0.05). As a result, the extrahepatic manifestations should.be carefully followed in patients with chronic hepatitis B and C infections, for an early diagnosis and appropriate treatment.
